- SOW Technical Evaluation Criteria This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 2011-96039 and is issued as a request for proposals (RFP), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-46. The associated North American Industrial Classification System (NAICS) code for this procurement is 541940 with a small business size standard of $7.0 million. Proposals are due by 7/1/2011. The place of performance shall be CDC Animal Care Facilites, Fort Collins, Colorado. The period of performance shall be a base period of 12 months with four 12 month option periods. Responsible sources that believe they possess the expertise and capabilities identified in the Statement of Work are encouraged to submit their technical and pricing proposal in the format they choose. Please forward the proposals to Maria Shamburger, Procurement and Grants Office, 2920 Brandywine Rd, Atlanta, GA 30341 or via email to heg9@cdc.gov by July 1, 2011, Reference 2011-96039. Questions may be submitted via email until June 17, 2011. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all proposals. The following provisions apply to this commercial solicitation: 1. FAR clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. 2. FAR clause 52.212-2, Evaluation -- Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. 3. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 4. FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. 5. FAR clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. 6. Additional contract requirement(s) or terms and conditions may be required for this acquisition if determined necessary by the contracting officer and consistent with customary commercial practices.
