- Past Performance Questionnaire Solicitation RFP DOL111RP20359 Contracting Office Address Department of Labor, Employment Training Administration, Division of Contract Services, 200 Constitution Avenue, NW, Room N-4655, Washington, DC, 20210, UNITED STATES Description The Department of Labor, Employment & Training Administration (DOL/ETA), Office of Foreign Labor Certification (OFLC) is soliciting proposals to select a contractor to provide Scanning & Digitizing Document Management services. The scope of this contract is to scan, digitize and store permanent and temporary nonimmigrant case files located at the OFLC Atlanta and Chicago NPC's and provide a web- based interface for authorized users to access and retrieve imaged case documentation 24/7/365. Per FAR 19.804-3 this procurement has been approved for 8(a) competition by the SBA and is restricted to 8(a) competition per FAR 19.805 Competitive 8(a). Per FAR 19.805 -2(b) the SBA will determine the eligibility of the initial Offerors, and the SBA will determine the eligibility of the successful Offeror established by the DOL per FAR 19.805-2(b) (2). Requests for Clarifications (RFC)/Questions are to be submitted electronically via E- Mail to Clyde Trumbore at Trumbore.Clyde@dol.gov, by 3 :00p.m., Tuesday June 21, 2011. Only electronic submissions of requests will be accepted. Should any RFC be received after the date stated above, the Government reserves the right not to provide an answer. If, however, the Government determines that a late RFC raises an issue of significant importance, the government will respond. All responses will be provided electronically Past performance evaluations shall be submitted by sending the Past Performance Questionnaire (Attachment A) to at least three (3) references that the offeror currently has or had a contract with within the past 5 years. The offeror shall instruct the reference to complete the questionnaire and E-mail it directly to Clyde Trumbore, Trumbore.Clyde@dol.gov. The completed PPQs must be received by the government by 2 P.M, Tuesday, July 5 th, 2011.
