- Statement Of Work This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Anticipated award date is 27 June 2011. The request for quote number is N6847012RQBL019 is issued as a Request for Quotation(RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular(FAC) 2005-50 dated 16 May 2011. NAICS Code for this requirement is 325413. The intention is to award a Firm-Fixed Price contract. The U.S. Naval Hospital, Okinawa, Japan intends to negotiate a sole-source contract with Haemonetics Corporation for the following service; CLIN:0001 PM & Upgrade Kit of Cell Washer qty 11 ea Manufacturer Haemonetics Corporation, ACP 215 Systems, Model#52963-00. See attached Statement of Work (SOW) for a detail. CLIN:0002 Travel & Accommodation Expenses qty 1 Lt Cost shall inludes all the expenses related to the work such as air fares, accommodations & transportations. However, any responsible source that can meet the requirements may submit the written notification to the Point of Contact (POC). Supporting documents must be furnished in sufficient detail, to demonstrate the offeror's capability to provide and ability to comply with the requirements. Offerors responding to this notice must be FDA approved for the manufacture, sale and distribution of these products. Offerors must be registered in Central Contractor Registration. [Note: Lack of registration in the Central Contractor Registration will make a Contractor ineligible for award. Registration is available via the Internet at www.ccr.gov. The place of performance will be directly to U.S. Naval Hospital, Okinawa, Japan, BLDG. 6000, Camp Lester, Chatan-Cho AZA Kuwae, Okinawa, Japan 904-0103 and completed by end of September 2011. The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) TECHNICAL: Offers must meet the specification in the solicitation and by subsequent amendments. (2) PAST PERFORMANCE, (3) Price. Quotes must be submitted to the POC no later than 27 June 2011, 11AM; Japan standard time. Telephone inquiries or requests will not be accepted. Quotes shall be submitted via e-mail in Word, Excel, or PDF format to the POC, Fumikazu Higa at fumikazu.higa@med.navy.mil or faxed to 011-81-98-893-5754. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, U.S. Naval Hospital, Okinawa, Japan will proceed with the sole source procurement.
