- Amendment 0002 is hereby issued to make a correction to the Section L provision entitled "Instructions, Conditions, and Notices to Bidders". The contents of Amendment 0002 reads as follows: 1. Amendment 0001 will not be issued. 2. Amendment 0002 is issued to correct the language contained in the Section L provision entitled "Instructions, Conditions, and Notices to Bidders". 3. Section L provision entitled "Instructions, Conditions, and Notices to Bidders", Paragraph 4e(3)(e) within Request for Proposal (RFP) W58RGZ-11-R-0003 states: "For all contracts identified in the "Contract Descriptions Section" of Volume III, Performance Risk Assessment Questionnaires shall be completed and submitted. The offeror and/or key subcontractor shall complete ONLY Part I of the Performance Risk Assessment Questionnaire, e-mail the questionnaire to both the Government contracting activity and technical representative responsible for the past/current contract, and provide the PCO with a list of all the POCs who were sent a questionnaire within 10 days after release of the RFP. The POC List shall be submitted in Word for Windows Table Format to include the following fields: Solicitation Number; Company Name; Contract Number; Government Agency; POC Last Name, First Name; POC Title; POC Telephone Number; POC E-Mail Address; Date E-Mailed to POC (month/day). Within the e-mail providing the questionnaire, the POC's shall be instructed to electronically complete Part II of the questionnaire and e-mail the entire questionnaire to the PCO designated in the RFP within 10 calendar days of receipt of the questionnaire." 4. Amendment 0002 to RFP W58RGZ-11-R-0003 revises Section L provision entitled "Instructions, Conditions, and Notices to Bidders", paragraph 4e(3)(e) to read as follows: "For all contracts identified in the "Contract Descriptions Section" of Volume III, Performance Risk Assessment Questionnaires shall be completed and submitted.
