- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Combined Synopsis/Solicitation (CSS) for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. The announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotations (RFQ) under Solicitation Number W81YG10730002 which will result in a Firm-Fixed Price contract. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-51, Effective 31 May 2011 and Defense Federal Acquisition Regulation Supplement (DFARs), current to DPN 20110606 (Effective 06 June 2011) Edition. This action is solicited as a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 333512 with a size standard of 500 employees. The United States Army Yuma Proving Ground (USAYPG), Yuma, has a requirement for Torchmate X CNC Plasma/Oxy Fuel Cutting Engraving Table System. Quotes are being solicited on a Brand Name or Equal Basis pursuant to FAR provision 52.211-6. The manufacturer of the brand name item being solicited is Applied Robotics, Inc., 280 S. Rock Blvd, Suite 150 Reno, NV 89502. Offerors proposing to furnish other than the brand name item being solicited shall provide sufficient technical literature to enable the government to determine that the proposed item meets or exceeds all minimum salient characteristics of the brand name being solicited. Delivery shall be made to the United States Army Cold Regions Test Center Bldg 605 Arctic Avenue Delta Junction/Ft. Greely, Alaska 99737. It is anticipated that payment will be made by Government Payment Credit Card. Offerors shall account for any costs associated with accepting credit card payment. All prospective bidders must be actively registered in the Central Contractor Registration (CCR). Offerors may register online at https://www.bpn.gov/ccr/default.aspx. Quotes are being solicited on a Lowest Price Technically Acceptable (LPTA) basis. Offers shall be clearly marked with RFQ referencing number W81YG10730002 and emailed to the Point of Contact (POC) listed below or sent by facsimile to 928-328-6534 no later than 27 June 2011 at 1:00 PM Mountain Standard Time (MST). In addition, all technical questions concerning this requirement must be emailed to the POC listed below no later than 20 June 2011 at 11:00 AM MST. As a minimum, offerors shall include with their quotation the following information: (1) Company name, address, Dun & Bradstreet Data Universal Numbering System (DUNS) number, and Tax Identification Number (TIN); (2) POC with telephone and facsimile numbers and email address; (3) Complete the annual representations and certifications at the Online Representations and Certifications Application (ORCA) website (https://orca.bpn.gov/), and submit a signed copy of FAR Clause 52.212-3(b). (4) Proposed price. Arizona vendors are to include the Arizona Transaction Privilege Tax; (5) Shipping needs to be incorporated into proposed price. Offerors that fail to furnish required representation or technical information as required by FAR provisions 52.212-1 or reject the terms and conditions of this solicitation may be excluded from consideration.
