- Description
- The Phoenix Area Indian Health Service (IHS) is soliciting proposals for professional psychiatric physician services to be provided on-site for patients of the Ft. Yuma Indian Hospital located in Winterhaven, California under a non-personal service contract. The contractor shall provide two clinic days of service on site for a total of 24 clinic days per year; it is expected that a total of 16-20 patients shall be see during each clinic visit. All potential offerors shall be classified as a business entity. In addition, all potential candidates shall be subject to pre-security clearances prior to commencing services at the government installation in Winterhaven, California; the security clearance involves fingerprinting and screen check through the Office of Inspector General (OIG) website. The anticipated period of performance is for a base year of twelve (12) months, beginning September 1, 2011 through August 31, 2012, with option to extend services for four (4) additional one-year option periods, or a total of five years. The Contractor shall meet all criteria contained in the Request for Proposal (RFP). The unit price shall be an all inclusive cost to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, including all other costs pertinent to the performance of the contract.
