Description
Wage Determination Contract Clauses Price/Cost BIOHAZARD WASTE PICKUP This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Request for Quotation (RFQ) are being requested and a written solicitation will not be issued. The current solicitation number is 11-247-SOL-00021 and is issued as a RFQ. This acquisition is being conducted under Simplified Acquisitions Procedures in accordance with FAR part 13. The clauses and provisions referenced in this solicitation may be obtained in full text form at http://www.arnet.gov/far. The Department of Health and Human Services/Indian Health Service has a requirement for Hazardous Waste Retrieval and Disposal service located at the Whiteriver Service Unit Whiteriver, AZ. A Firm-fixed price with economice price adjustment contract is anticipated Base + 4 option years anticipated. A. Hazardous Waste Retrieval and Disposal Service Contract I. SCOPE OF WORK: The Contractor shall perform the Biohazard waste retrieval and disposals for the Indian Health Service Whiteriver Service Unit. The project is a component of the Department of Health and Human Services. The initial contract will be one (1) year, with options years to extend. II. DESCRIPTION: This service contract is for the Biohazard Waste Retrieval and Disposal for the Indian Health Service Whiteriver Service Unit. Procedures shall include, but not limited to, the following: 1) Shall provide all necessary supplies and equipment to provide the required services. 2) Shall provide pickup, transport, and disposal services for all infectious waste including chemotherapy waste. 3) Shall provide clean, rigid containers to the Government for use in storing infectious wastes between pick-ups. "BIOHAZARD" and/or the international "biohazard" symbol shall be visible on all the containers. Provide appropriate containers for hazardous waste, chemo waste and pharmaceutical wastes etc. 4) Shall ensure the transport vehicle has appropriate placards to carry biohazard materials. The transport vehicle shall be fully enclosed with lockable doors and sealed to preclude leakage of liquids from the enclosed areas. The transport vehicle should provide unrestricted access to the infectious waste containers for purposes of loading and unloading. 5) Transporter: The driver will adhere to all posted speed limits in the Government compound and use extreme caution when maneuvering and parking the transport vehicle during pickups. All the labor to remove filled containers from secured storage area, loading the vehicle and replacing with empty containers in secured area is the sole responsibility of the transporter/driver. 6) Updates: any and all updates pertaining to the Biohazard waste retrieval and disposals service or any changes to the contractor's policy and procedures shall be communicated to the Contract Officers Technical Representative (COTR) and the Contractor with confirmation from both parties of changes or updates. III. GOVERNMENT FURNISHED MATERIAL/FACILITIES: Biohazard barrels will be provided by the Government. The barrels will be stored and maintained in a secured and locked enclosed fenced area. The biohazard (red) trash liners will be provided by the government. IV. DOCUMENTATION/REPORTS: On each service date, the contractor will provide a receipt of the barrels collected & loaded including the type of barrels i.e. hazardous waste, and/or chemotherapy waste with the COTR. V. CONFORMANCE STANDARDS: Contractor shall comply with applicable Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), conform to Federal, Tribal, State and local regulatory guidance and ordinances in regards to bio hazardous waste, disposal and applications. VI. PERIOD OF PERFORMANCE: "Tentative Schedule" July 1, 2011 thru June 30, 2012 The initial contract will be one (1) year, with four (4) option years. VII. SCHEDULE OF DELIVERIES/HOUR OF OPERATION: The Biohazard Waste Retrieval and Disposal will be picked-up once every week between 6am and 8am, excluding federal holidays. Scheduled service date is to be arranged between the designated COTR and the contractor. 1)Federal Holidays: New Year's Day, Marin Luther Kind Day, Presidents day, Memorial Day, Independent Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. Also any other day declared by the President of the United States to be a national holiday. The Government may exercise its option to extend the term of this contract, but not to exceed four (4) years. This option may only be utilized when contract awards are fully priced for the second, third and fourth years at the time of the award. All responsible business may submit a quote including unit pricing information, the prompt payment discount terms, Dun & Bradstreet Number (DUNS), Taxpayer Identification Number (TIN), their certification of business size, valid registration in the Central Contractor Registration (CCR) www.ccr.gov.
