- Attachment # 7 - NQF Submission Form This purpose of this amendment for Solicitation No. AHRQ-11-10007-A is to clarify the following items: 1. The Past Performance Information due date is added to Page 1 of the Solicitation: Date Past Performance Information Due: July 7, 2011 2. The following sentence is added to Page 1 of the Solicitation: The number of awards AHRQ intends on making depends on the number of proposals received. AHRQ anticipates making up to 12 awards. 3. The number of copies on page 1 of the Solicitation part A. is changed to read: "Technical Proposal (See Section L.9) (1 original and 9 Copies, plus two electronic copies on CD )." 4. The number of pages for the Technical Proposal referenced on Page 1 of the Solicitation is changed to read: "Your technical proposal must be concisely written and should be limited to 100 typewritten pages (double-spaced), inclusive of personnel qualifications (i.e., resume, etc., see Section L.10 for additional details). 5. Page 15 of the Statement of Work - Section C.4.1 Analytic data capacity, bullet 4 is changed to read: "Registry and surveillance data such as the National Surgical Quality Improvement Program and National Healthcare Safety Network." 6. Page 78 of the Solicitation part III is changed read ( FOR INFORMATION ONLY is removed ): SMALL DISADVANTAGED BUSINESS PARTICIPATION PLAN: See Small Disadvantaged Business Plan Instructions for format (L.11). 7. Page 82 part B of the Solicitation under Engaging Stakeholders and Supporting Transparency is changed to read: "Demonstrate methods to utilize or develop transparent methodologies in measure development work." 8. Section L.10 Past Performance Information on page 85 is changed to read: A list of up to five (5) relevant contracts and subcontracts completed during the past three years and all contracts and subcontracts currently in process that are similar in nature to the solicitation workscope. 9. The last paragraph in Section L.10 on page 86 is changed to read: The attached Past Performance Questionnaire and Contractor Performance Form ( Attachment #1 ) shall be completed by those contracting organizations listed in (1) above. 10. Section L.11 Part B. is removed from the RFP. 11. Attachment 7 - National Quality Forum Submission Form (13 pgs) has been added to the Solicitation and is attached to this amendment.
