Description
1. Background The DAF contains 61 fire doors as noted on the attached WPI-001 Checklist A. Work was performed on some of these fire doors. We need to know whether these 61 fire doors can retain their certification as fire doors. 2. Purpose We need a certified fire door inspector to conduct a visual inspection of these 61 DAF fire doors and door frames to determine whether these fire doors can retain their certification as fire doors. Inspections will be conducted IAW NFPA 80 and NFPA-101. Any deficiencies discovered will be so noted along with suggested action to correct these deficiencies. Visual inspection will be followed by appropriate labeling of the fire doors which retain their fire rating reflecting this new inspection. We have allotted two days for this visual inspection. A second visual inspection will be required of any DAF fire doors which have been repaired. We have allotted one day for this visual inspection. This visual inspection will also be followed by appropriate labeling of these repaired fire doors. Visual inspection of these fire doors will be at the DAF located approximately twenty miles from Mercury, Nevada. Visual inspections will be supported by the appropriate NNSS personnel [Millwright & NSTec Fire Marshall as a minimum] using a work package to document the scope and results of these inspections. 3. Requirements / Specifications a. CONTRACTOR will provide SUBCONTRACTOR with copies of the latest revision of the following drawings: 006-DAF-300-A1 006-DAF-300-A2 006-DAF-310-A3 [with corrections noted] 006-DAF-331-A2 006-DAF-332-A2 006-DAF-370-A14 006-DAF-400-A4 006-DAF-611-A3 006-DAF-711-A2 006-DAF-712-A2 006-DAF-DOORS-3AA 006-DAF-DOORS-3BA b. SUBCONTRACTOR shall use checklists included in NFPA-80 and NFPA-101. SUBCONTRACTOR may use additional checklists for visual inspections if deemed necessary by the SUBCONTRACTOR. c. SUBCONTRACTOR will provide to CONTRACTOR, in writing, results of inspections per the checklist(s) and other observations made during the jobsite walk down/inspection of conditions that may need correction in order to satisfactorily certify the DAF fire doors. SUBCONTRACTOR shall submit written report to CONTRACTOR within one week of inspection. d. SUBCONTRACTOR and CONTACTOR will reference work items by DAF door number. e. SUBCONTRACTOR personnel performing inspections and labeling shall be qualified in accordance with NFPA-80 & NFPA-101 and SUBCONTRACTOR applicable procedures. f. SUBCONTRACTOR will be escorted to and from the DAF Entry Guard Station by the CONTRACTOR for security and safety purposes. g. SUBCONTRACTOR will follow all NSTEC Radiological Control Requirements. These requirements will be set forth in the work packages or otherwise presented/briefed to all personnel. h. SUBCONTRACTOR will follow all NSTEC Security Requirements as written in Exhibit F Security Requirements document. See attached Exhibit F document 4. Deliverables: a. SUBCONTRACTOR shall provide CONTRACTOR with copies of all certificates of qualifications of personnel to NFPA requirements one week prior to start of work. b. SUBCONTRACTOR will provide final Reports within one week after visual inspection has been performed. Remember that two certification inspections of DAF fire doors and their associated frames are required. One inspection is required before any repairs are performed. Then a second inspection will be performed after repairs are completed.
