Description

Triathlon PS TKA 5 each Accolade Stem; Tritanium Cup 2 each Notice to Contractor: Solicitation number N00259-11-T-0291 is issued as a notice to procure from only one source IAW FAR 13.106-1(b). This proposed contract action is used for Orhopedics Department for Total Hip Arthroplasty and Total Knee Arthroplasty surgeries, consumables are compatible and specific to the patients of Naval Medical Center San Diego that is schedule for implant on May 27th 2011. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is Stryker Orthopaedics, CAGE Code: 9H084, NAICS: 339112. Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that Stryker Orthopaedics device is the only company that manufactures and distributes the supplies that is compatible with equipment currently used by NMCSD. IAW FAR 5.207(c)(15)(ii), all responsible sources may submit a quotation, which shall be considered by the agency. The Government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-51. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov and/ or www.acq.osd.mil. Please fax (619-532-5596) or email your quote to Merlinda Labaco at merlinda.labaco@med.navy.mil on or before 05/26/2011 10:00 AM PST. Email submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions.
