Description
DESCRIPTION - This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This RFQ and incorporated provisions and clauses are those in effect through the latest Federal Acquisition Circular (FAC). The solicitation number for this request is F11PS00827, and the solicitation is issued as a Request for Quotation (RFQ). This solicitation is set aside for Small Business. The NAICS 212321 - Construction Sand and Gravel Mining with a size standard of 500 employees. Quotation will only be accepted from approved Small Businesses. The Government intends to issue a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation will be most advantageous to the Government. Factors used to evaluate offers shall be: lowest price of the offers. Prices must include all costs for furnishing all labor, materials and equipment necessary for the supply and delivery of the gravel & soil products. REQUIREMENT - The U.S. Fish and Wildlife Service, San Bernardino National Wildlife Refuge has a requirement for 950 TONS of Rip-Rap Rock, size 4"-8" and 2,000 TONS of Clay-Based Soil, meeting the requirements to line pond bottoms to conserve water infiltration. PLACE OF PERFORMANCE - San Bernardino National Wildlife Refuge, Douglas, Arizona, 85607. BID SCHEDULE 1.CLIN 0001: Rip Rap Rock - Quantity 950 TONS, sized 4" - 8" 2.CLIN 0002: Clay Based Soil - Quantity 2,000 TONS Delivery of the subject products MUST be completely by September 30, 2011 Prices must include all costs for furnishing all labor, materials and equipment necessary for the supply, delivery and installation of the subject products. DELIVERY (FOB) - The delivery location is on the San Bernardino National Wildlife Refuge and shall be delivered and dumped to a site as directed by the Refuge Manager, Bill Radke 520-364-2104 x101. LEGAL OBLIGATIONS - The contractor shall be responsible for the observance of law ordinances, statues and regulations pertaining to the hauling of the gravel and soil products. RULES AND REGULATIONS - The contractor shall conduct work in the conformance with such appropriate Refuge regulations as the Manager may prescribe, and shall also conduct the operation as not to interfere with or endanger the ordinary use of structures, roads, utilities, and other Refuge facilities. Equipment operations and materials shall be confined to the limits defined by the U.S. Fish and Wildlife Service. Special attention should be given to the protection of facilities, wildlife and vegetation adjacent to the work areas. PROVISIONS AND CLAUSES - In addition, FAR provisions/clauses listed below apply. QUOTATION SUBMISSION - The deadline for receipt of quotes is June 10, 2011 at 3:30 pm (Mountain). Quotes may be submitted on letterhead and must include, at a minimum, unit price for each item, extended price, item description as detailed above, prompt payment terms, name, phone number and address of your point of contact. Quotations may be submitted by e-mail, fax or any mail delivery service.
