- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in the Federal Acquisition Regulations (FAR) subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Solicitation No. is VA-241-11-RQ-0429 and is issued as a Request for Quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-37. The North American Industry Classification System (NAICS) number is 339113, the business size is 500 employees. Consistent with FAR 6.302.1, Only One Responsible Source, the Contracting Office at the Department of Veterans Affairs - Boston Healthcare System intends to issue a sole source award to Medrad, for the following items for a base and four option years: STATEMENT OF WORK Medrad disposable supplies are used in the Boston Veterans Affairs Medical Centers. The complete list of items are listed below. Medrad disposables described below are exclusively designed for the MedRad Stellant Injectors, Possis AngioJet Ultra Thrombectomy System, and the Spectris Solaris MR injection system that is used in three Cats Scan, Interventional Radiology and MRI respectively at VA Boston Healthcare System. No other disposables fit these injectors and thrombectomy system; only those made by Medrad. The CT stellant syringes and swabble sets are used in the contrast/saline injectors in cat scan. The console catheter sets are used with the AngioJet Ultra Thrombectomy System used in interventional radiology for declotting of bypass grafts, AV acess conduits, peripheral arteries and peripheral veins. The tubing and spectris solaris syringe kit are used in the contrast injectors in MRI. The Department of Veterans Affairs, VA Boston Healthcare System is requesting the following line items manufactured by Medrad, One Medrad Drive, Indianola, PA 15051. Estimated workload based on FY10 history. The Government is looking for a FIRM-FIXED PRICE for all items from date of Government signed contract until the end of the contract period. Base year plus four option periods. These quantities will cover the Boston Veteran Affairs Medical Centers.
