- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, N4155711RC071FS, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-16. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 423910 and the small business size standard is 100 employees. The following commercial items are requested in this solicitation: CLIN 0001- (Qty 25 EA) Life Fitness 17 inch Attachable TV-NTSC/ATSC P/N: LCD-0201-07 - BRAND NAME OR EQUAL. Unit Size: 17.3 inch length x 12.1 inch width x 28 inch diagonal; Screen Size: 17 inch; Screen Type: LG Philips LCD Panel; Tuner Type: Digital/Analog; Viewing Angle: 170 degrees horizontal and 170 degrees diagonal. BRAND NAME OR EQUAL items must include all requested parts, part number and measurements/sizes and must be compatible with Life Fitness Equipment. CLIN 0002- (Qty 25 EA) Universal Bracket T-02 OR BRAND NAME EQUAL. Elevation television bracket and iPod Cord to fit 17 inch LCD Universal Mounting Bracket. CLIN 0003- (Qty 25 EA) TV Remote Control IR-TV-REMOTE OR BRAND NAME EQUAL. Remotes must be compatible with Lifefitness TV monitor listed above. CLIN 0004- (Qty 1 EA) Shipping to Tracy, CA 95376 CLIN 0005- (Qty 1 EA) Shipping to Yigo, Guam 96929 Requested Items are brand name OR EQUAL. Equal items must include all requested parts and measurements along with catalog cuts/pictures to be technically acceptable. Items should be shipped 30 Days ADC, FOB: Destination for delivery to 36 FSS, FPO AP, Andersen AFB Guam,. PLEASE NOTE: PROPOSALS SUBMITTED "IN CARE OF" ANOTHER COMPANY WILL NO LONGER BE ACCEPTED. ALSO, BIDS SUBMITTED "ON BEHALF OF" ANOTHER COMPANY WILL NO LONGER BE ACCEPTED. The following provisions and clauses apply to this acquisition: FAR 52.211-6 Brand Name or Equal. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. Quotes will be evaluated and awarded to the offeror with the lowest price technically acceptable. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, and the offeror must include a completed copy of this provision with their proposal. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items.
