- Solication RFQ SP330011Q1002 Attachment 5 Attachment 4 Attachment 3 Attachment 2 - SOW DDOO Attachment 1 - SOW DDRT This Request for Quotation (RFQ) is being issued to establish a firm-fixed price contract for the purchase and installation of a Brand Name or Equal Rite-Size box makers at Defense Logistics Agency Distribution Red River, Texas (DDRT) and Defense Logistics Agency Distribution Oklahoma City, Oklahoma (DDOO). Contractor shall provide all materials, supervision, tools, installation, labor, training, and materials to install and make operational the corrugated box maker in accordance with the Statement of Work (SOW) provided at Attachment 1 and Attachment 2. In addition, Contractor shall provide all transportation and rigging services to off-load, move, position, install each corrugated box maker within a designated location of Building. DDRT currently has two (2) Rite-Size box makers (Models BMC36 and BMC55) and DDOO has one (1) Rite-Size Model RS-84, which will accept as a trade-in of the older model toward the purchase of the newer model. ( SEE Attached combined synopsis/solication and SOW) This acquisition is 100% small business set-aside basis. The North American Industry Classification System (NAICS) code for this project is 333291. The small business size standard is 500 employees.
