- A revised solicitation is attached including Questions & Answers. Important note, the # of days has changed from 27 to 26 estimated days. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation is available on the FBO website. RFQ Number W912PQ-11-T-0027 is being issued as a Request for Quote. The solicitation documents and incorporated provisions are those in effect through Federal Acquisition Circular 05-26. This procurement is unrestricted. The cost magnitude is estimated to be $500,000 and $1,000,000. The NAICS code applicable to this RFQ is 621210. Under the solicitation, the contractor is required to provide an on location mobile dental practice with the capability to provide examinations to approximately 740 New York Army National Guard Personnel. The place of performance will vary throughout the state through the end of the fiscal year. At this time there are 3 events scheduled, Camp Smith Training Site, the Watervliet Arsenal, and Fort Drum, NY MATES. All technical requirements pertaining to this solicitation are contained in the Statement of Work. The provision at 52.212-1, Instructions to Offerors - Commercial applies to this solicitation. Award will be made to the lowest responsible offeror that is technically acceptable. The provision at 52.212-3, Offeror Representation and Certifications apply to this solicitation. Interested parties must complete the Representation and Certifications at the ORCA website. Go to www.bpn.gov/orca to complete this requirement. Central Contractor Registration (CCR) is mandatory for award consideration. Go to www.ccr.gov to register. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
