- Statement of Work for Barrier Maintenance and Repair Services This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) FA4877-11-Q-0138 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-51, Defense Circular 20110427 and Air Force Acquisition Circular 2011-0421. (iv) This procurement is being issued as a Small Business Set-Aside under NAICS code 238990 with a Small Business Size Standard of $14 million dollars. (v) The contractor shall provide all personnel, labor, transportation, supervision, materials, equipment, and management: to provide the following: Barrier Maintenance and Repair Services through 30 Sep 11, Davis-Monthan AFB, AZ 85707. Please refer to the attached statement of work for additional information and requirements. Note: A site visit will be conducted at Davis-Monthan Air Force Base on Thursday 19 May 2011 at 9:00 A.M. PST. All interested contractors shall respond in advance with their intent to attend this site visit by contacting SrA Mitchell Johnson at e-mail mitchell.johnson@dm.af.mil, Phone (520) 228-2377, or Fax (520) 228-5462, NLT 1:00 P.M. PST Monday 16 May 2011. The place of initial meeting for the site visit will be at the Craycroft Gate (Visitors Center) NLT 8:45 A.M. PST. All personnel planning on attending and driving on the installation will need valid state driver's license, current proof of vehicle insurance and registration. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price. (x) Offerors must include a completed copy of the provision at, 52.204-8, Annual Representations and Certifications and submit with its offer. (xi) Clause 52.204-9, Personal Identity Verification of Contractor Personnel - contractor shall comply with agency personal identity verification procedures identified in the contract that implement Homeland Security Presidential Directive-12 (HSPD-12), Office of Management and Budget (OMB) guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201.
