- This is a combined synopsis/solicitation for commercial services and prepared in accordance with the format in Subpart 12, as supplemented with additional information included in this notice. This acquisition is unrestricted and this announcement constitutes the only solicitation. Proposals are being requested and no written solicitation will be issued. The solicitation number is VA-257-11-RP-0214 and is issued as a request for proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50. VISN 17, VA North Texas Health Care Systems (VANTHCS) located at 4500 S. Lancaster Road, Dallas, TX 75216 has a requirement for Adult Day Home Services provided to Veteran beneficiaries in the North Dallas and Richardson, Texas area. Facilities located in Dallas County and Collin County would be considered for evaluation/inspection. The Contractor shall be a Medicare/Medicaid certified facility or receive special approval from the Geriatrics and Extended Care Strategic Healthcare Group. The Contractor shall provide Adult Day Home care services, to Veteran Beneficiaries, at the contractor's facility. The Contractor shall register in Central Contractor Registration (CCR) at the website https://www.ccr.gov and the ORCA website at https://www.orca.bpn.gov This is a competitive Request for Proposal (RFP) which may result in multiple awards. The solicitation incorporates by reference FAR clauses and provisions: 52.212-1, 52.212-4, and 52.212-5. Additionally VAAR 852.203-70, 852.216-70, 852.233-70, 852.233-71852.237-7, 852.237-70, 852.270-1, 852.271-70 are applicable to this requirement and can be accessed electronically at the following address: http://farsite.hil.af.mil/vfvara.htm. The applicable NAICS code is 623110 and the SBA size standard is $13,500,000. Proposals are due no later than 2:00 p.m. Central time on May 16, 2011. Award will be made based on the best value offered to the Government.
