- Attachment # 4 - Price Schedule Attachment # 5 - Site Visit Reservation Form Attachment # 3 - Wage Determination (Rev 9) Attachment # 2 - Past Performance Questionnaire Attachment # 1 - Performance Work Statement Solicitation for Military Family Housing Maintenance at Ellsworth AFB - FA4690-11-R-0003 (26 April 2011) Ellsworth Air Force Base South Dakota has a requirement for Military Family Housing Maintenance. The contractor shall provide and maintain the required Military Family Housing Maintenance in accordance with the attached soliciation. The Contract Manager and alternate(s) and all employees must be able to read, write, speak, and understand English. There is a Small Business set-aside for this procurement. This will be a Past-performance/ Price Tradeoff procurement using Source Selection Procedures. The NAICS code for this project is 561210 with a size standard of $35.5 Million. The classification code is 'S'. The period of performance is 01 August 2011 through 31 March 2012 with an additional two 6-month option periods. There will be a site visit on 9 May 2011 at 9:00 AM MST. To attend this site visit, e-mail Attachment # 5 - Site Visit Reservation Form to angel.pearce@ellsworth.af.mil. Offerors not already registered in the Central Contractor Registration (CCR) database are highly encouraged to do so by visiting the site, http://www.ccr.gov, or by calling 1-888-227-2423; an offeror must meet this requirement to be eligible for contract award.
