- Amendment to RFQP05051100009 The Federal Bureau of Prisons, Federal Medical Center Carswell, Fort Worth, Texas, intends to issue solicitation RFQP05051100009 to make a single award to a responsible quoter for the provision of Bio-Medical Waste Storage and Disposal Services. The contractor shall pick-up, transport, and dispose of the bio-medical waste. The pick-up and transportation will be provided during the normal duty hours of this institution (Monday - Friday, 8:00am - 2:00pm). FMC Carswell will operate on the Federal Holiday schedule in which contract services will reschedule accordingly. The required number of pick-ups will vary, depending on the amount of Bio-Medical Waste. 1. Bio-Medical Waste / storage boxes. 2. Heavy gauge, low density, red plastic bags, appropriately labeled "Bio-Medical Wastes." 3. All necessary report forms for pick-up and disposal of Bio-Medical Wastes in full compliance with all state and federal regulatory agencies concerning the pick-up transportation and disposition of Bio-Medical Wastes. Provide manifest documentation within 7 working days of the pick-up and disposal of the Bio-Medical Waste. FMC Carswell will supply the following: 1. An appropriate site for the storage of Bio-Medical Waste until pick-up. The successful contractor must be licensed by the State of Texas and comply with all EPA, Federal, State, and Local laws and statutes regarding pick-up and disposal of Bio-Medical Waste. The rate for these services shall be on a per box basis. Quotes per box shall include all charges for supplies, pick-up, transportation, handling, disposal, and other related fees. Award shall be made to the responsive/responsible bidder submitting the lowest Total Bid Amount. It is estimated that approximately 40 boxes of Bio-Medical waste is generated per month. The contract period shall consist of one (1) twelve (12) month base year and four (4) twelve (12) option periods for renewal at the discretion of the Government. The North American Industrial Classification (NAICS) code is 562112, with a small business size standard at $12.5M. The solicitation will be available on or about April 21, 2011 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http://www.fedbizopps.gov/. Hard copies of the solicitation document will not be available. The site provides downloading instructions. All future information about this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. All qualified sources are encouraged to submit an offer which will be considered for award. Faith-Based and Community-Based Organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible. The estimated date for receipt of offers is May 16, 2011. A site visit is scheduled for May 09, 2011 at 9:00am CST for all interested quoters. A background clearance will be required in advance to the site visit.
