- DESCRIPTION: The Department of Veterans Affairs Nebraska Western Iowa Health Care System (NWIHCS) intends to negotiate a commercial (FAR Part 12) sole source contract with CHF Solutions, INC, 7601 Northland Drive, Brooklyn Park, MN 55428, under the authority of FAR 6.302-1. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) FAC 2005-50. Only one responsible source and no other supplies or services will satisfy agency requirements. A justification for other than full and open competition is being prepared in support of this acquisition. The requirements of this solicitation are as follows: A1100, Aquadex Aquapheresis System (W/Hematocrit)1 Each A1110, Aquadex Console Cart1 Each ESP002, Silver (Extended) Service Plan, 2 Years1 Each Disposables A2000, UF-500 Set with Hematocrit (5 to box)1 Box A1664, Coiled Dual Lumen Extended Length Catheter (cdELC) (5 to box)1 Box THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. NAICS Code 339112, with a size standard of 500 employees applies to this procurement. All inquiries received by 3:00 P.M. Central Time, Friday, April 29, 2011, will be considered by the Government. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.
