Description
Statement of Work (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is 20056703 and is issued as a request for quote (RFQ) Submit written offers only, oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-48. (iv) The associated North American Industrial Classification System (NAICS) code for this procurement is 622110; Small business size standard is $34.5M. This procurement shall be full and open competition. All qualified, responsible sources may submit an offer that shall be considered by the agency. Offerors are advised that they are responsible for obtaining amendments. (v) The Department of Homeland Security (DHS), Customs and Border Protection (CBP), Indianapolis, IN. intends to award a blanket purchase agreement for the procurement of emergency medical services for CBP detainees IAW attached STATEMENT OF WORK (vi) The contractor shall be responsible for providing all labor, equipment, personnel, licenses, permits, supplies and supervision. It is required that all contractors submitting a quote review the attached STATEMENT OF WORK. All contractors submitting a quote shall be required to meet all the requirements found in the STATEMENT OF WORK. (vii) Requested services shall be performed at a medical facility within a 10 mile radius of the CBP Port of Laredo. (viii) The provision at FAR 52.212-1 Instructions to Offerors-Commercial Items, applies to this acquisition. (ix) The Government will award a contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government, price and other factors considered. Award shall be based on All or None and Best Value to the Government. Best value can include but is not limited to price, delivery date, and technical capability. (x) The clause at FAR 52.212-3 Offeror Representations and Certifications - Commercial Items, applies to this acquisition. Complete only paragraph (j) of this provision if the annual representations and certificates have been completed electronically at http://orca.bpn.gov. If the annual representations and certifications have not been completed electronically at the ORCA website, then only paragraphs (b) through (i) of this provision need to be completed. (xi) The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. FAR 52.212-4 addendum is also included. (xii) FAR 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items, and the following clauses in paragraph (b), (c); 52.219-14, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.223-18, 52.232-33, 52.239-1,.
