This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and supplemented with additional information included in this notice. The solicitation number is VA-244-11-RQ-0170 and it is issued as a Request for Quotation (RFQ).This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation is 100% set-aside for Veteran Owned Businesses. This Solicitation is subject to the Service Contract Act. Note: Wage Rates are attached The NAICS code is 238220. Successful offer will be the one that provides the Lowest Price that is Technically Acceptable. The VA Pittsburgh Healthcare System has a requirement for the service listed below. All work will be performed at our facility located at University Drive C, Pittsburgh, PA 15240. All communication regarding quotes, questions, etc. shall be submitted by email to Russell Eury (Russell.Eury@VA.Gov). Phone calls will not be accepted. Quotes shall be submitted by email by 2:00 PM, Eastern Time, on Tuesday, May 3, 2011. There will be one site survey for interested parties to view the proposed work area. Interested parties that would participate in the site survey shall meet @ VA Pittsburgh Healthcare System University Drive C in BLDG 1 in conference room (AN219) on Monday, April 25, 2011 at 09:00 AM EST. Quotes shall be submitted by email. Please read the entire solicitation. Offerors are responsible for all terms, clauses, and conditions contained therein. The following provisions/clauses apply to this solicitation: FAR 52.212-1 Instructions to Offeror--52.212-3Offeror Representations and Certifications-Commercial Items (JAN 2011)--FAR 52.212-4 Contract Terms and Conditions Commercial Items--FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items--52.204-9 Personal Identity Verification of Contractor Personnel --52.228-5 Insurance-Work on a Government Installation -- 52.212-2 Evaluation-Commercial Items--852.219-11 VA Notice of Total Veteran-Owned Small Business Set-Aside -- 852.246-70 Guarantee -- 852.237-70 Contractor Responsibilities -- 852.273-76 Electronic Invoice Submission --52.211-6 Brand Name or Equal --804.1102 Verification of Status of Apparently Successful Offeror --52.232-38 Submission of Electronic Funds Transfer Information with Offer --FAR 52.204-7 Central Contractor Registration (vendor's registration in CCR must be current and complete to be considered)--VAAR 852.203-70 Commercial Advertising East Tank Coating Scope of Work GENERAL: 1.Contractor will provide all labor, material, equipment, supervision and other resources as necessary to accomplish re-lining and other work described below for the East Tank (horizontal) potable domestic water storage tank with approximate dimensions 12'-6" diameter x 32' long. 2.The re-line system shall require the application of TNEMEC Porta Pox Plus (Series N 140) or equal epoxy coating at 4-5 mils per coat ((1) mil primer and (2) 4mil /ea coats). The coating must comply with AWWA C 102 systems No.1 and No. 2 and AWWA C 210. Note: The minimum Dry film thickness is 12 mils. ANSI/NSF 61 standards for potable water service. 3.The East Tank is located on the Ground Floor in Building 1, University Drive Division of the VA Pittsburgh Healthcare System. 4.VAPHS will provide access to temporary electrical power panels in work area for contractor equipment. Contractor is responsible for installing and maintaining load connections from panels per all applicable building and electrical codes. 5.Contractor must review job site to determine material, supplies and equipment staging and access locations for support equipment, e.g. vacuum truck, sand blast pot, etc. including cables, tubes, hoses, etc. VAPHS must approve of locations. No parking will be provided for contractor personal vehicles. 6.Contractor must take all steps necessary to prevent dust, fumes and noise from affecting hospital areas outside the immediate tank area. Steps may include installing environmental controls and working outside regular business hours. 7.Contractor is responsible for unloading any equipment and materials delivered to job site. 8.Contractor responsible for proper transport, storage and disposal of bitumastic material removed from tank. 9.New tank lining must be guaranteed to remain intact and protective for five (5) years from installation completion. 10. Disinfect the cured coating in accordance with AWWA C652 standards. SUBMITTALS: 10.Contractor must submit proof of adequate insurance prior to commencing work. 11.VAPHS must approve project schedule to be submitted by contractor prior to commencing work. 12.Contractor must submit a confined entry program for VAPHS review and approval. Program is to be approved by VAPHS prior to commencing work. a.Contractor program must meet requirements of VAHPS Memorandum EC-042: Confined Space Program (attached). b.Contractor is responsible for all tank entry arrangements, i.e. VAPHS will provide no support or assistance with tank entry. c.Contractor must submit OSHA training records including 10-hour and confined space entry training prior to commencement of work. This includes applicable respirator training/approval. 13.Submit MSDSs for all chemicals and liner materials proposed for use. SITE PREPARATION: By VAPHS: 14.Drain the tank prior to arrival of contractor By Contractor: 15.Prior to commencing work and inspections inside the tank, remove all piping connections to tank. This is to comply with "Non-Permit Entry" procedure in VAPHS Memorandum EC-042. Contractor responsible for reconnecting all piping after re-lining work and tank entries are completed required to place tank in service. 16.Install temporary environmental controls to prevent dust and fumes from going beyond the immediate tank area 17.Install necessary confined space entry equipment, e.g. ventilation controls, retrieval system, etc. RE-LINE TANK: 18.Abrasive blast clean the tank interior surfaces per SSPC-SP5, removing all existing coating, rust and scale. Remove all blast debris in closed containers from the work site and dispose of into dumpsters in the same manner as a non-hazardous waste. 19.Follow all manufacturer requirements to prepare tank surface and apply TNEMEC Porta Pox Plus (Series N 140) epoxy coating or equal epoxy coating (ANSI/NSF 61-approved tank lining which conforms to Pennsylvania Code 109.606) to the interior of the tank. 20.Coat man-ways, man-way covers, flange faces and other appurtenances to ensure that all surfaces that water can potentially contact have been coated. 21.Re-lining material application is to be either by hand by trained applicators or by alternate application method approved by manufacturer, as needed to meet manufacturer's requirements for lining thickness consistency and adhesion to interior tank surfaces. New lining must be installed in one continuous application. 22.Work area must be maintained in a neat, clean and orderly manner throughout all phases of the job. PLACE TANK BACK IN SERVICE: 23.Rinse interior of tank to remove all traces of contaminants and debris. 24.After the coating has cured, perform a low voltage holiday test on the applied lining to assure a pinhole free installation. 25.Smooth all bumps, drips and "stalactites" from coated surfaces, particularly mating surfaces, e.g. flange and man-way faces. 26.Contractor responsible for cleaning and decontaminating tank.
