Attachment C - Past Performance Evaluation Questionnaire Attachment B - Contractor Past Performance Evaluation Attachment A - Statement of Work SF 1449 Request for Proposal 11-258-SOL-00009 The National Hansen's Disease Programs (NHDP) has a requirement for Social Worker Case Management services for patients with Hansen's Disease treated at the National Hansen's Disease Programs, Baton Rouge LA. Services include: educational, evaluative, treatment, and referral services in relation to the psychosocial needs of the patients and residents of NHDP; psychosocial assessments; provide individual, family, group, and crisis counseling; determine eligibility for social service programs, make referrals to community agencies on behalf of and with the permission of the patients and their families, and document the services performed in patient records; provide social work intervention to high risk patients; organize weekly Clinical-Rehabilitation Staffing meetings; participate in discharge planning meetings and coordinate discharge with other departments, patient families, and other outside agencies; make travel arrangements for patients to and from the NHDP facility and assess and assist with the financial needs of the patient during treatment; and, assist with the preparation of advance directives such as living wills, provision of a surrogate decision-maker, and plans for burials and disposition of personal belongings. The anticipated period of performance is one year, beginning July 1, 2011 through June 30, 2012 with a one year option. This is a 100% small business set-aside, restricted to small businesses eligible to receive award. The RFP will be posted at the Government website at www.fbo.gov under HHS, Health Resources and Services Administration, BPHC/National Hansen's Disease Programs, Posted Dates on or about April 19, 2011. It is the offeror's responsibility to monitor the site for amendments to the solicitation. The NAICS code for this requirement is 624190. The small business size standard is $7.0 million. Solicitation will be conducted under Federal Acquisition Regulation (FAR) Part 12 and 13. The closing date for receipt of proposals will be approximately thirty (30) days after issuance/posting of the RFP.
