Description
The purpose of Modification 2, to Solicitation W91LV2-11-MobileDental, is to add a fourth Operation Ready Warrior Event (ORW) in Fort Chaffe, AR from 16 July - 21 July 2011 (Statement of Work (SOW) attached), post revised SOW's for the two (2) Fort McCoy and one (1) Fort Hunter-Liggett Events (sections 1 and 1.1 have been revised on all three (3) SOW's), add additional language pertaining to FAR 52.212-1(h) and FAR 52-212-2(a), post additional information pertaining to questions asked by vendors, and to extend the response date to 4:00pm Eastern Standard Time (EST) on 22 April 2011. Modification 2 also deletes FAR 52.228-5 from the Solicitation. Additional Federal Acquisition Regulation (FAR) Language: IAW FAR 52.212-1(h), the Government reserves the right to make multiple awards resulting from this solicitation provided that all prospective awardees do not place any restrictions to do so on their offers and if it is in the best interests of the Government. Consequently, offerors may bid on any one or a combination of any of the following Groupings of CLINs: Grouping I: Fort Hunter Liggett, CA: 06 May - 28 June 2011 CLIN 0001: Four (4) each Mobile Dental Practices CLIN 0002: Three (3) each Mobile Dental Practices CLIN 0003: One (1) lot Dental Supplies Grouping II: Fort McCoy, WI: 03 June - 25 June 2011 CLIN 1001: Three (3) each Mobile Dental Practices CLIN 1002: One (1) lot Dental Supplies Grouping III: Fort McCoy, WI: 07 July - 27 August 2011 CLIN 2001: Three (3) each Mobile Dental Practices CLIN 2002: One (1) lot Dental Supplies Grouping IV: Fort Chaffe, AR: 16 July - 21 July 2011 CLIN 3001: One (1) each, Mobile Dental Practice CLIN 3002: One (1) lot Dental Supplies Proposed language for Evaluation Criteria for 52-212-2(a): In addition to other evaluation factors, bids will be evaluated on the basis of advantages and disadvantages to the Government that might result from making more than one award (multiple awards). It is assumed, for the purpose of evaluating bids, that $500 would be the administrative cost to the Government for issuing and administering each contract awarded under this solicitation, and individual awards will be for the items or combinations of items that result in the lowest aggregate cost to the Government, including the assumed administrative costs. Additional Information (pertaining to questions asked by vendors): - Dentists and assistant personnel are not required. However, on ground IT and clinical operation support is needed. Coverage will need to be adequate enough to be able to provide assistance to multiple locations that may be 30 - 40 minutes apart, on a given installation. - Additional insurance riders for liability for the Mobile Dental Practices will not be required. - Waste (both solid and fluid) will be handled by the contractor. Additionally, all energy requirements are the responsibility of the contractor. - Question: Mobile dental practices will utilize the Apteryx commercial digital radiographic software that is comparable to the U.S. Army Dental Command (DENCOM) Digital Enterprise Viewing & Acquisition Application (DEVAA). At the end of each event, all x-ray images will be delivered on an external storage device to the USAR personnel in the DICOM format. Our software does export images to DICOM; however, we do not have the Apteryx software. Will our software work as long as we are able to deliver images on an external storage device in the DICOM format? Answer: Yes, images in DICOM format from non-Apteryx software will be acceptable. - All operatories must have water/sink capabilities. - Portable buildings are acceptable as long as they are able to meet mission requirements. They must be flexible for location changes without impact on clinical availability. - The term operatory(ies) applies to the delivery of treatment, including exams, and corrective procedures. Reusable equipment and supplies will be in sets for such treatment. The contractor must provide all necessary equipment and supplies to sterilize durable equipment and supplies. - IT support will need to be on site from 0730-1700, with 24-hour accessibility for issue resolution. A single individual may cover a given location, with multiple mobile dental clinics on different training areas. - If portable buildings are used, the buildings will not require ramps/lifts. - All supplies and equipment are not disposable. Routine readiness treatment will be provided to Soldiers, to include fillings and possible simple extractions. - Multiple training exercises will be occurring in the same location, with different populations. The desire outcome is to have enough supplies not to interrupt clinical procedures, (mainly exams) during Soldier in-processing to the installation. - The type of dental chair required is one that "can move in an automatic self-propelled vertical direction and swivel in a horizontal position without obstruction, i.e. no field or hand powered dental chairs". This requirement is firm - no similar type chairs will be acceptable. DEADLINE: Quotes MUST be received by 4:00pm Eastern Standard Time (EST) on 22 April 2011. Quotes are to be emailed to joseph.haslach@us.army.mil or faxed to Joseph Haslach, Contract Specialist, at (609)562-2920. If the quote is faxed, the contractor MUST send an email to joseph.haslach@us.army.mil within 30 minutes of the fax being sent, stating the time the fax was sent, to eliminate the possibility of the quote not being received do to errors in the faxing process. An official authorized to bind your company shall sign the quote. Quotes must contain all information requested or will not be considered for award.
