Description
The United States Army Garrison - Miami (USAG - Miami) Department of Emergency Services (DES) is hereby soliciting information about potential sources to conduct Occupational Health Screening services for Department of the Army Civilian Police (DACP) and Department of the Army Security Guards (DASG) personnel. The Contractor shall provide the requested services as condition of employment, pre-employment, fit for duty, individual reliability program (IRP) at the Contractor's location, in accordance with (IAW) Army Regulation (AR) 190-56. The Contractor shall conduct physical examinations, L-urinalysis test, audiogram test, visual acuity test, electrocardiogram test, physical performance test and mental health evaluations on an annual basis to DACP and DASG. The Contractor shall manage medical records, provide clinical laboratory services, operate fitness facilities and programs, and maintain all occupational health related equipment. The Contractor shall provide written results of all test provided to DACP and DASG and complete Part C of the Optional Form (OF) 178 (Please see attached sample form) to the USAG - Miami DES. Interested offerors having the required specialized capabilities to meet the above requirement should submit a capability statement of ten (10) pages or less indicating their ability to perform all aspects of the effort described herein. Responses must include the following: name and address of firm, size of business; average annual revenue for past 3 years and number of employees; ownership; whether they are large, small, small disadvantaged, 8(a), HUBZone, and/or woman-owned; number of years in business; affiliate information: parent company, joint venture partners, potential teaming partners, prime contractor (if potential sub) or subcontractors (if potential prime); list of customers covering the past five years (highlight relevant work performed, contract numbers, contract type, dollar value of each procurement; and point of contact - address and phone number). One of the primary goals of this synopsis is to assist the U.S. Government in determining whether an 8(a) or other small business set-aside is an acceptable strategy for this procurement. If a full and open competition is ultimately pursued, responses to this synopsis will be used to aid in establishing small business subcontracting goals. Since no decision on a set-aside has yet been made, all qualified firms are encouraged to respond. The North American Industry Classification System (NAICS) Code for this procurement is 621498 and the size standard under this code is $10 million. No solicitation exists; therefore, do not request a copy of the solicitation. If a solicitation is released it will be synopsized in FedBizOpps. It is the potential offeror's responsibility to monitor this site for the release of any solicitation or synopsis. Technical and procurement related questions should be directed to SSG G. J.
