Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 246-2011-Q-0005 for Medical Waste Pickup and Disposal services. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50. The NAIC code is 562211 and the small business size standard is $12.5M. Medical Waste Pickup and Disposal services Period of Performance April 1, 2011 or date of award through March 31, 2012 Evaluation will be based on price and past performance Price Schedule for Waste Disposal Services Waste Disposal Service for 12 months at $________ per month = $___________. Past Performance is ability to provide documentation of past performance of satisfactory service performed in a federal facility or hospital. The offeror shall provide past performance references with name, contact name, period of performance and telephone number. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. STATEMENT OF WORK: The Lawton Indian Hospital and the Anadarko Indian Health Service requires a nonpersonal professional service to be performed onsite at the following locations: Lawton Indian Hospital, Housekeeping Department 1515 Lawrie Tatum Road Lawton, Oklahoma 73507 Phone: 580.353.0350 Anadarko Indian Health Center 115 NE Old Town Anadarko, Oklahoma 73005 Phone: 405.247.2485 Contractor shall provide all labor, equipment, tools, materials, and transportation necessary to provide medical waste pick-up and disposal at the above referenced locations. Period of Performance is April 1, 2011 through March 31, 2012. The schedule for medical waste pickup and disposal are as follows: Lawton Indian Hospital - Once per week on Monday Anadarko Indian Health Center - Every other week on Wednesday Request Preferred Program or other that will include medical waste pickup/disposal with complete documentation such as manifests and tracking documents, flat monthly fee determined on stops/year/daily stops, OSHA Compliance manual, blood borne pathogens trainings, quarterly safety meetings, and onsite annual OSHA training. The Contractor shall provide leak proof containers with liners, 4.0 cu ft, 28-30 gallon, reusable containers. The Contractor shall provide required tracking method of labeling of each container for transport and incineration of hospital waste with copies to the Lawton Indian Hospital Housekeeping Department, Rickie Mithlo, Wage Leader, 580-354-5295. The Contractor shall provide and show proof of insurance coverage to protect the United States Government against all claims arising from handling of the waste from the Lawton Indian Hospital and Anadarko Indian Health Center to the point of incineration. The Contractor shall provide to the Lawton Indian Hospital a letter indicating the employees working for the Contractor has had adequate training in handling "Medical Waste". The Contractor shall provide handling of waste until incineration in a manner not reflected back to the Lawton Indian Hospital. The Contractor shall only use specially approved vehicles for transport of medical waste from the Lawton Indian Hospital. The Contractor will provide training for the health care workers in determining what are "Medical Waste" and the handling of such waste. LAWTON Contractor to pick-up and dispose of medical waste for the Lawton Indian Hospital of twenty three (23) - sixty (60) TB01, 28-30 Gallon reusable, 4.0 cu ft containers located on campus, once a week on Mondays. Containers are the property of the contractor. Containers picked-up will vary each month. Total yearly estimated pickup for the year is 950 - 2100. ANADARKO Contractor to pick-up and dispose of medical waste for the Anadarko Indian Health Center of one (1)-ten (10) TB01,28-30 gallon, reusable, 4.0 cu ft containers located at clinic, once every other week on Wednesdays. Containers are the property of the contractor. Containers picked-up will vary each month. Total yearly estimated pickup for the year is 136-200. Weekly in Medical Waste Tracking Form with manifest number will be left at the Lawton Indian Hospital, Housekeeping Department for Lawton and Anadarko. A courtesy copy of the original monthly invoice should be sent to Lawton Indian Hospital Housekeeping Department, fax# 580-354-5264. Accepted Waste for pick-up and disposal: Sharps (needles, syringes, scalpel blades, glass pipettes, slides, etc.) Laboratory Wastes (Cultures and stocks of Biohazard Level 1, 2, and 3 infectious agents, test tubes) Contaminated Disposal Material, Equipment, and Instruments (includes but is not limited to: blood administration sets, drainage collection devices, disposal gloves and gowns, dressings) Dialysis Tubing and filters (Dialyzers) Blood, Blood Products, Body Fluids Accepted Waste which maybe identified and segregated for Incineration: Trace-contaminated Chemotherapy Waste (empty drug vials, syringes and needles, spill kits, IV tubing and bags contaminated gloves and gowns, and related materials as defined in Federal and State standards. Pathological Waste (Human or animal body parts, organs, tissues and surgical specimen (exclusive of formaldehyde or other preservative)
