Description
THIS IS NOT A SOLICITTION, REQUEST FOR PROPOSAL (RFP), INVITATION FOR BID (IFB), REQUEST FOR QUOTE (RFQ), OR REQUEST FOR APPLICATION OF A SOLICITATION, AND DOES NOT ASSUME THE GOVERNMENT WILL AWARD A CONTRACT OR PROCEED WITH ANY OF THE ABOVE SOLICITATIONS IN THE NEAR FUTURE. THIS NOTICE IS A SMALL BUSINESS ONLY SOURCES SOUGHT REQUEST FOR MARKET RESEARCH PURPOSES. This program is intended to assist employers in establishing comprehensive workplace health programs (WHP). The program will support 70-105 small, mid-size, and large employers through a contract with a firm with the expertise and capacity to plan, develop, and administer a core workplace health program targeting employee risk for physical inactivity, poor nutrition, weight, and tobacco use. This initiative will use evidence-based and best practice workplace programs, policies, practices, and environmental supports to maximize employee engagement and participation in the wellness program; raise employee awareness and education around health; and establish a work environment to change unhealthy behaviors and lifestyle choices to reduce employee risk for chronic disease. A separate contract will provide for a national evaluation of the program. Goals: 1) Reduce the risk of chronic disease among employees and their families through evidence-based workplace health interventions and promising practices; 2) Promote sustainable and replicable workplace health activities; 3) Promote peer to peer healthy business mentoring. The core tasks for the contract will include: 1) Recruit and secure the commitment of employers forming 7 cohorts across the country of 10-15 employers each to participate in the program; 2) Demonstrate organizational commitment and support of worksite health promotion at all levels of management in each employer in the cohort; 3) (Deleted) 4) Establish baseline data for each worksite setting in the cohort to enhance the sustainability of the workplace health program over time; 5) Implement all elements of a core workplace health program that are evidence-based, sustainable and replicable in all worksites in the cohort; 6) Promote peer to peer healthy business mentoring among participating employers in the cohort; 7) Participate in activities to bring broad-based support to the national network of cohorts, facilitated by CDC, which includes technical assistance and training, communications, and a national evaluation of the program. The minimum contractor requirements include: 1) The Contractor shall have demonstrated experience in developing, implementing, administering and evaluating public health-oriented workplace health programs in companies of various size and industry sectors; 2) Eligible offerors must have a national/regional reach into employers on the state/local levels; 3) The Contractor must be able to create and adapt culturally and linguistically appropriate and scientifically accurate messages and materials through multiple communication channels, targeted to both English and non-English-speaking audiences, and for employees with low health literacy levels; 4) Eligible offerors must have experience developing and implementing workplace health protection and promotion interventions equivalent to that of NCQA or URAC health and wellness certified firms; 5) The Contractor must have proven successful experience working with state or local level public health agencies and/or community-based health organizations. CAPABILITY STATEMENTS MUST DEMONSTRATE THE MINIMUM REQUIREMENTS OUTLINED ABOVE. Capability statements, not to exceed seven pages in length, shall include the following information: company name, address, point of contact, phone/fax/email, and business size and status (e.g. small business, 8(a), veteran-owned small business, service-disabled veteran owned small business, HUBZone small business, small disadvantage business, and women owned small business). The NACIS code for this requirement is 541612. Small Businesses having the capabilities necessary to perform the stated requirements may submit capability statement via email to ins4@cdc.gov. The subject line of the email shall be "Comprehensive Wellness Capability Statement." Responses are due not later April 10, 2011. Capability statements will not be accepted after the due date.
