- Experience Questionnnaire (page 7 & 8 of packet) Amended Solicitation correcting the spelling on page one from 'Ferrier' to 'Farrier'. Also re attaching the experience questionnnaire. This is a combined synopsis/solicitation for a commercial service item prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Solicitation number AG-03R6-S-11-0020 is issued as a Request for Quotation (RFQ). This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. The clauses and provisions referenced in this solicitation can be found in full text format at https://www.acquisition.gov/far/. This is a total small business set aside and the small business size standard is $7 million. The North American Industry Classification Code is 541990. A quotation is requested for Shoeing of horses and mules for a total of 100 head (work to be preformed 4 times per year/25 each) and Trimming services of horses and mules for a total of 11 head (work preformed once a year). Work to be done at the Ninemile Ranger District, 20325 Remount Road, Huson, MT 59846. Work to be preformed between the 15th of April, 2011 through the end of September, 2011. The awareded Purchase Order will be a firm fixed price contract for service. The Western Montana Acquisition Zone (WMAZ) serves as the Forest Service contracting office for Western Montana and is located in Missoula, MT. The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price, experience and past performance considered. In addition, offerors will include with their offer a completed copy of the FAR provision 52.212-3, Offer Representations and Certifications - Commercial Items. Offeror may print a copy of their ORCA registration to meet this provisions requirements and submit with offer. The FAR clauses 52.212-4, Contract Terms and Conditions - Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Execute Orders - Commercial Items, apply to this acquisition. Quotes must be received by March 31, 2011 at 4:00 P.M. Mountain Standard Time at the Lolo National Forest, Supervisor's Office, Attn: Contracting, Building 24, Fort Missoula, Missoula, MT 59804.
