- Amendment 1 with Amended Attachment 4, Past Performance Survey A second amendment addressing vendor questions is forthcoming. Any new information will be posted to this site. Thank you. The purpose of amendment 00001 is as follows: 1. To extend the proposal submission date from March 16, 2011 at 2pm Eastern Time to April 4, 2011 at 2 pm Eastern Time; 2. To incorporate changes in the Past Performance Survey. New fields to capture administrative data have been added. References who have submitted their surveys are not required to resubmit their information on the revised form. All others should use the revised form included with this amendment. The revised Attachment 4 is attached. 3. To extend the due date for Past Performance surveys. This due date has been extended from March 15, 2011 at 1pm Eastern Time to March 31, 2011 at 1pm Eastern Time. 4. All other terms and conditions remain unchanged.
