Description
This is NOT a solicitation announcement. This is a request for information only. The purpose of this sources sought synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled Veteran-Owned, Veteran-Owned Small Business, Hubzone, 8(a), small, small disadvantage and large business) relative to NAICS code 811219 (size standard of $19.0 million). Responses to this request for information synopsis will be used by the government to make appropriate acquisition decisions. Responses to this sources sought shall NEITHER constitute responses to solicitation which is not currently available NOR a request to be added to a prospective bidders list to receive a copy of the solicitation. The decision to solicit for a contract shall be solely the Government's discretion. The Michael E. DeBakey VA Medical Center is seeking information for a potential contractor to provide comprehensive preventive maintenance, inspection, repair, and software update services to OEM specifications on the following equipment: The Cyberknife ® Stereotactic Radiosurgery System manufactured by Accuray, Inc. The comprehensive services shall include but not limited to scheduled and non-scheduled maintenance, inspection including calibrating if needed of the systems, repair including replacement of broken or malfunctioned parts with new parts, and updating the software applications currently installed in the systems whenever the manufacturer of the systems releases the updates. The contractor must be an authorized factory trained service provider for the above equipment and must provide evidences of OEM parts and materials replacement. Sources interested in this notice must submit the following information in writing: Required Information: 1.Company name, address, point of contact, DUNS No. registered in Central Contractor Registration (www.ccr.gov), cage code, size of business pursuant to North American industrial classification system (NAICS) 811219. 2.Evidences of authorized factory trained service provider for the above equipment and evidences of OEM parts and materials replacement. 3.Evidences of having access to obtain replacement parts, materials, and software updates from the manufacturer of the Cyberknife ® Stereotactic Radiosurgery System. 4.List of customers (Names, primary points of contact, and phone number/email addresses) with whom the contractor has had or currently has contracts for providing preventive maintenance and repair services to the Cyberknife ® Stereotactic Radiosurgery System. 5.and responses to the following questions: (a)Is your firm registered in Central Contractor Registration: www.ccr.gov. (b)Is your business large or small business under NAICS 811219? (c)If small, is your firm qualifed as a small, emergent business, or a small, disadvantaged business? (d)If disadvantaged, specify under which disadvantaged group and is your firm certified under section 8(a) of the small business act? (e)Is your firm a certified "hub zone" firm? (f)Is your firm a woman-owned or operated business? (g)Is your firm a certified service-disabled veteran owned and veteran-owned small business? If yes, is your firm registered at http://www.vip.vetbiz.gov/general_user/search/default.asp and verified by the VA's Center for Veterans Enterprise (CVE)? This is not a request for proposals or request for quotes and in no way obligates the government to award any contract. Responses to this notice shall be in writing by e-mail addressed to Namphuong.nguyen@va.gov. All responses must be received in writing no later than 4:30 PM, local time on March 9, 2011. Fax or telephone responses will not be accepted. It is the responsibility of sources that decide to send the responses to make sure that the designated office receives their information. Please be advised that the designated office can receive the following electronic file format: Microsoft Word, Microsoft Excel, and PDF. Please also be advised that the designated office's email cannot receive any attachments exceeding 5MB. It is the responsibility of the interested sources to ensure the designated office to receive the complete files by either reducing the file size or breaking it down into smaller attachments. Firms responding to this announcement, which fail to provide all of the required information, cannot be used to help the government make the acquisition decisions, which is the intent of this synopsis. This notice is to help the VA medical center in determining potential sources only. No solicitation is currently available. All firms responding to this request for information notice are advised that their response to this notice is not a request to be considered for a contract award or to be added to prospective bidders list to receive a copy of the solicitation.
