- Justification & Approval Letter (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FM448610180005; this solicitation is a Request for Quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-48. (iv) There is no small business set-aside for this acquisition. This acquisition will be full and open competition; the North American Industry Classification System (NAICS) code is 423450. The business size standard in number of employees is 100. The following commercial items are requested in this solicitation: (v) CLIN: 0001- Visual Field Plotter 750I, HFA 750i w/table including USB, DICOM Gateway 2.0, DG 2.0 license, NetPro, GPA, SITA SWAP & Kinetic; (QTY 1 EACH) Item shall be received 45 days ADC FOB: Destination (vi) See paragraph (v). (vii) Place of Delivery: Lajes Field, Azores, Portugal. The following provisions and/or clauses apply to this acquisition. (viii) Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items. If quoting an "or equal", a specification sheet or other information must be submitted that provides sufficient detail to determine whether or not the substituted products can satisfy the need. (ix) FAR 52.212-2 Evaluation -- Commercial Items (Jan 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
