February 16, 2011 Lisa P. Nooner Bay Pines Veterans Affairs Healthcare Systems 10, 000 Bay Pines Blvd. Saint Petersburg, Florida 33708 The Department of Veterans Affairs, Bay Pines Veterans Healthcare Systems, Bay Pines, Florida intends to purchase 1-Flo-Lab, 1-DICOM for PACS and 1-17" Touch Screen Monitor. Open Market materials listed below are required. Interested parties may identify their experience in providing the open market materials and services to complete the procurement of these items to the Contracting Officer via e-mail at lisa.nooner@va.gov or fax to 727-399-3346. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number, VA-248-11-RQ-0194, is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43 effective July 23, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 339112, Size Standard: 500 Employees. This procurement is unrestricted and must be able to quote on all or none. 1.Flo-Lab 2.
