Description
Description: This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification; (Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Small Business Small Disadvantage Business, Woman Owned, Hub zone, 8(a), or large business) relative to NACIS 624190 (size standard of $7 million) The Louis Stokes Cleveland VA Medical Center is seeking potential sources to provide professional Art Therapy services two (2) four (4) hour days, eight (8) hours per week, between 8:00am-4:30pm; plus supplies. Treatment provided will include all necessary supplies except adaptive equipment. Adaptive equipment will be provided by the SCI OT/PT department. All Art Therapy services will occur on site at the Louis Stokes Cleveland Department of Veterans Affairs Medical Center (LSCDVAMC), SCI Center, Wade Park. Services shall be provided for the VA Spinal Cord Injury (SCI), Hospice, and Day Hospital patients who are receiving either inpatient or outpatient care. The specific groups to be targeted for treatment: 1) Inpatients on extended bed rest for a chronic condition 2) Outpatients that have been identified as high risk for psychological secondary complications from SCI and 3) Inpatient Palliative care and Hospice patients. Art Therapist(s) must have a Master's Degree in Art Therapy. Clinical supervision shall be provided through the Contractor. Credentialing through the Art Therapy Credentialing Board is desired, but not mandatory. This contact is effective from the date of award, with four (4) one (1) year options. The contract will be subject to the availability of funds. Submittal Requirements: Responses to this notice should include company name, a service capability statement, three customer references to which you have provided similar services to, copies of applicable licensing and credentialing, product catalog, address, point of contact, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (4) is your firm a certified hub-zone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a Certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business through the VIP Database? Responses to this notice shall be e-mailed, mailed or faxed to the attention of Maladean E. Hudson Contracting Officer, The Louis Stokes Cleveland Department VA Medical Center, 10000 Brecksville Rd, Brecksville, OH, 44141; Fax: 440 717-2842; e-mail: maladean.hudson@va.gov. Telephone inquires will not be accepted. Responses must be received in writing no later than February 15, 2011, 9:00am. This notice is to assist the VA in determining potential sources only. A solicitation is not currently available. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought announcement, a combined synopsis solicitation announcement will be published in FEDBIZOPPS. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the response to this announcement. Responses to this sources sought synopsis are not considered adequate responses to any future solicitation announcements. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of future solicitations.
