- Q - Autopsy Services NAICS Code: 621399 Solicitation Number: Notice Type: Sources Sought Synopsis: This is a sources sought notice only; it is not a request for quotations or offers. The Fargo VA Medical & Regional Office Center is conducting market research to identify potential sources for a contract to provide autopsy services conducted on as needed basis within 50-miles of the Fargo VA Medical & Regional Office Center located at 2101 Elm Street (04-S), Fargo, ND 58102. The contract period will be for one (1) year from the date of contract award plus four (4) one (1) year options, to be exercised at the discretion of the government. The North American Industry Classification System Code (NAICS) is 621399 with a size standard of $7.0 million (USD). This procurement will be conducted in accordance with VA Acquisition Regulation (VAAR) Part 873 in conjunction with Federal Acquisition Regulation (FAR) Parts 12 and 15. This sources sought notice is to assist the Fargo VA Medical & Regional Office in identifying an appropriate acquisition strategy only. Written communication received will be considered solely for the purpose of determining currently available. If a solicitation is issued, it will be announced and posted on this website at a later date, and interested parties must respond to the solicitation to be considered for award. INTERESTED PARTIES SHALL RESPOND NOT LATER THAN TUESDAY, FEBRUARY 22, 2011 AT 2:00 PM (CENTRAL TIME) All interested parties should insure they are registered in Central Contractor Registration (CCR), the primary registration database for the U.S. Federal Government. Other website of interest for contractors is http://www.orca.bpn.gov To be considered for award, the successful offeror must be registered in Central Contractor Registration (CCR) and provide the following information 1.A company profile, including name of business, DUNS number, and other pertinent information 2.A description of capabilities. 3.Past performance data for similar size facilities, multiple locations which demonstrate your capability to satisfactorily execute a contract to provide safe and timely laboratory courier services as described in this notice. Data shall include customer name, address, contract number, and point of contact information (name, phone number, and e-mail address.) 4.The business size and business socioeconomic category; such as Service-Disabled Veteran Owned Small Business, Veteran-Owned Small Business, Hub Zone Small Business, etc ¦ Please note, all SDVOSB's and VOSB's must be verified by the office of Small Disadvantaged Business Utilization's Center for Veterans Enterprise (CVE) prior to receiving any award. This notice does not commit the government to contract for any supply or service. The government will not pay for any information or administrative cost incurred in response to this Request for Information or Solicitation for Planning Purposes.
