- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W91YTV-11-T-0077 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-49. Federal Acquisition Regulation (FAR) Clause. This requirement is 100% Small Business Set-a-side and the North American Industry Classification System Code (NAICS) for this procurement is: 339115; and the small business size standard is 500. The Optical Fabrication Laboratory at Moncrief Army Community Hospital, Fort Jackson, SC, 29207 has a a need for Optical Support Services for a base period plus two (2) one-year option periods, if exercised. The Contractor shall provide the service of adequate personnel inclusive of a Team Leader to support a full service multi-vision optical fabrication laboratory in all production phases of the lab, where single vision and multifocal spectacles are fabricated and or repaired on a production line.
