- This Sources Sought Notice is not a request for formal proposal. This Notice is to identify Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Businesses (VOSB) qualified as such under PL 109-461 who are able to provide commercial ambulette or non-emergency medical coach services 24 hours, 7 days per week to VA Beneficiaries of the Phoenix, Arizona Veterans Affairs Health Care System (PVAHCS) (650 E Indian School Road, Phoenix, AZ 85012-1892) and its seven community based outpatient clinics (CBOCs). Vendor shall furnish Wheelchair Type vehicles (Medi-Coachs). Services shall include vendor having an established facility(ies) in operation for at least one (1) year where ambulette units are housed and dispatched from a location within a thirty (30) mile radius of the PVAHCS' installations. Intended contract to be awarded on/about March 11, 2011. All responses shall include identification/certification of company size status and any pertinent information pertaining to this sources sought notice. This synopsis is for information and planning purposes and is not to be construed as a commitment by the Government. This synopsis is not a solicitation announcement for proposal and no contract will be awarded from this announcement. Responses shall be emailed to Frank Gamboa III, Contracting Officer @ frank.gamboa@va.gov. Period of performance will be April 1, 2011 through March 31, 2012 with four (4) one-year option renewals thereafter. NAICS code is 485991 (Special Needs Transportation) with small business size standard being $7.0M. NAICS 485991 Size Standard $7.0M. Due to time constraints the closing date for responses is February 11, 2011 @ 5:00 PM MST.
