- This amendment to the notice of intent to award on a sole source basis to the State of Utah issued on 1/28/11 is issued to identify a reduction in the amount of work required for this proposed contract. The requirements will only consist of collecting earnings estimates and W-2?s or other documentation to support year-end reconciliations from the remaining BOPD participants in the state. The requirements for work reports, assisting with final work continuing disability reviews (CDRs), and providing quarterly updates have been removed.
