- HQ0034-10-T-0091 Solicitation Amended February 7, 2011 HQ0034-10-T-0091 Performance Work Statement Revised January 31, 2011 with highlighted text of inserted or changed verbiage. HQ0034-10-T-0091 52.212-2 Addendum Revised February 7, 2011 which includes instructions to offerors and evaluation criteria Amendment dated February 7, 2011 to solicitation to HQ0034-10-T-0091 This amendment to the above referenced solicitation notifies potential offerors that the solicitation, Performance Work Statement, and evaluation criteria in FAR 52.212-2 Addendum for this solicitation have changed. In addition, a section titled "Special Contract Provisions" has been added. Additions or changes to the Performance Work Statement and evaluation criteria in FAR 52.212-2 Addendum have been highlighted in yellow. Deletions are not notated. Please refer to the original Performance Work Statement and evaluation criteria in FAR 52.212-2 Addendum of this solicitation for comparative purposes. Included in the FAR 52.212-2 Addendum changes is the addition of a transition period prior to the start of the new contract. The transition period must end prior to March 31, 2011 and may not exceed more than 15 days prior to April 01, 2011, the beginning of the base period. Plans for the transition period should be included in the technical proposal. The following changes to the PWS are affected under Amendment dated February 7, 2011: Sections 1.1, 1.3, 1.4, 1.6, Special Requirements - Liability, and Special Requirements - Permits and Licenses. The following changes to 52.212-2 Addendum are affected under Amendment dated February 7, 2011: Section/paragraph 3(a)(iii)- a) PART I - TECHNICAL PROPOSAL Section/paragraph 3(c)(ii)- c) PART III - PRICE PROPOSAL Section/paragraph 4(c) - Documents submitted in response to this RFQ... Section/paragraph 4(d)(v) - Documents submitted in response to this RFQ... Incinerator Support HQ0034-10-T-0091 52.212-2 Addendum Table, section L.1.a The Quote may be a total of eight (8) pages. CLINs ending in 3, i.e. 0003, 1003, 2003, 3003, and 4003 do not require a price. These CLINs are time and material CLINs and are reimbursed at actual cost. Government will establish a NTE amount. All other criteria for proposal submission remain the same. The deadline for submission of proposals for the solicitation referenced above is 10:00 a.m. EST Thursday, February 17, 2011.
