Description
The individual providing services under this purchase order, which involves regular contact with, or control over Indian children, is subject to a character investigation in accordance with the "Indian Child Protection and Family Violence Prevention Act" P.L. 101-630. The Contractor must perform in accordance with section 231 of the Crime Control Act of 1990, P.L. 101-647. Failure to comply with these statutes will result in immediate removal. 52.232-18 AVAILABILITY OF FUNDS (APR 1984) Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer Medical Support Assistant (DENTAL) shall furnish services for the Dental Department at the White Earth Health Center in Ogema, MN. The contractor shall provide services for Indian health service beneficiaries served by the clinic. This position will be used to cover summer vacation and training absences of civil servant employees. Therefore the work schedule will be flexible to accommodate the vacancies/SHORTAGES as they occur. Duty hours are eight (8) hours per day 8:00 am to 4:30 pm Monday through Friday with a ½ hour lunch break. Contractor shall work up to 5 days a week at 8 hours per day equaling 40 hours per week. SCOPE OF WORK FOR MEDICAL SUPPORT ASSISTANT (DENTAL) PERSONAL SERVICES CONTRACTOR WHITE EARTH HEALTH CENTER OGEMA, MN 56569 PURPOSE OF CONTRACT: The mission of the White Earth Health Center in Ogema, MN is to deliver quality care in order to attain positive health outcomes for all our valued patients with emphasis on cultural sensitivity and human dignity. To meet the mission, the White Earth Health Center requires Medical Support Assistants to support and assist in temporary duties assigned at the clinic. Individuals providing services under contract, which involve regular contact with, or control over, Indian children are subject to a character investigation in accordance with "Indian child protection and family violence prevention act" P.L. 101-630. The Indian health service personnel office will conduct these investigations following award of a contract unless in the judgment of the contracting officer, in consultation with the personnel officer, investigation may be waived based on prior investigations on file. Until the Character Investigation has been completed and the contracting officer notified, such individual should not have unsupervised contact with Indian children. The contracting officer has the authority to temporarily wave the character investigation of a contractor. The "Agreement to a Temporary Waiver of Character Investigation" authorizes an agency to hire a contractor prior to the completion of a background check provided that at all times the contractor is within the sight and under the supervision of a government employee who has successfully completed the national agency check with written inquires (NACI) Each contract resulting from this solicitation shall: (A) Be a fixed-price personal services contract under which the contractor is subject to supervision and direction of a designated government officer. (B) Allow the government to evaluate the quality of professional and administrative services provided. CONTRACTOR FURNISHED PROPERTY The Indian health service does not anticipate a need for contractor furnished equipment beyond a computer, desk, chair and office supplies. GOVERNMENT FURNISHED PROPERTY, FACILITIES, AND SERVICES The personal service contractor will be authorized to use all areas of the facility available to civil service and commissioned personnel in similar positions. The same restrictions apply to the personal service contractor and any other Indian health service personnel to use areas for official business activities only. Each personal service contractor shall be provided with an orientation to the service unit and the Indian community being served. Clinical personnel shall provide the orientation. The Contractor agrees to adhere to all applicable federal, state, and local laws and regulations as prescribed for safe operation and maintenance of the equipment and the completion of the sessions. The clinic shall provide all equipment and office supplies deemed necessary to provide administrative support at the clinic with the exception of those items to be supplied by the contractor as listed. The clinic shall provide all required blank government forms applicable to the Indian health service. Clinical personnel will also provide appropriate orientation to the necessity of such forms. The clinic shall provide an ongoing method of monitoring and evaluation of the quality of services performed before the level of performance becomes unacceptable. Contractor will be under the direct supervision of the Business Office Manager or designated representative. STATEMENT OF WORK: Medical Support Assistant (DENTAL) shall furnish services for the Dental Department at the White Earth Health Center in Ogema, MN. The contractor shall provide services for Indian health service beneficiaries served by the clinic. Contract performance shall be according to the requirements contained in this statement of work. The Business Office Manager or designated representative will be the direct supervisor for the contractor. The contractor shall advise his/her immediate supervisor or representative of any problem encountered in connection with meeting the needs of the Business Office Department. The contractor shall report continuing problems of any nature to the I.H.S. Project office officer. RESPONSIBILITIES: Medical Support Assistant (DENTAL) shall provide registration services for eligible patients (children, adults and elderly) at the White Earth Health Center. The Business Office Manager will monitor contractor activity. Specific Registration services shall include, but are limited to the following:  Serving as receptionist:  Receives and relays incoming and outgoing telephone and intercom messages.  Receives and directs patients and visitors, answers routine inquiries and makes appropriate referral of questions concerning patient's condition.  Performing recordkeeping duties:  Assembles patient records according to prescribed format.  Inserts additional forms in chart as necessary and stamps with correct patient identification.  Files results of treatment in medical/dental records.  Reviews charts of patients and records a variety of medical/dental data.  Performing clerical duties relating to patient care and treatment:  Determines patients' eligibility for treatment.  Records dentists' orders for patients on a variety of records.  Charts temperatures, blood pressures, respirations and other medical data.  Selects, completes and routes requests for patient activities and treatment procedures.  Enters test results and dental laboratory information on patient records.  Schedules appointments for patients with other medical services.  Management of Dental Clinic Schedule:  Manages the dental, clinic schedule, assuring appointments slots are filled, no shows are minimized, and cancellations are filled with waiting list patients immediately.  Uses the computerized schedule program to schedule patient's appointments at the time they present or call for an appointment.  Incumbent enters the monthly provider appointment schedule in the computer within two days of receiving the appointment schedule.  Incumbent follows policy and procedure for overbooks, cancellation and scheduling changes and works with the providers to ensure a well functioning scheduling program.  Incumbent maintains the scheduling program.  Incumbent will track no-shows and mail letters as appropriate.  Provides weekly appointment schedules to individual providers one week in advance.  Incumbent pulls dental records for the next day's appointments and the appropriate PCC is placed in the medical record. The medical records are in time order and by provider.  If the medical record is needed by the Dental Department the medical record will be requested through the Scheduling Package.  Incumbent adheres to the scheduling guidelines and they are kept up to date.  Performing miscellaneous duties:  Prepares patient census reports.  Maintains bulletin boards and library references.  Keeps time and attendance records.  Maintains a variety of information files.  Orders and keeps record of supplies.  Receives and distributes mail to medical staff and patients. QUALIFICATIONS: 1. Working knowledge of eligibility requirements of the IHS program. 2. Knowledge of whether documented proof offered meets IHS requirements. 3. Working knowledge of specialized medical terminology. 4. Knowledge of various clinical systems, walk-ins and appointments to avoid unnecessary delay/confusion on the part of the patient. Knowledge of the various services offered by each clinic. 5. Knowledge of interview techniques, Business office functions, policies and procedures to be able to refer inquiries to appropriate personnel. 6. Knowledge of internal organization structure and Medical Records in order to assemble patient information, to initiate new patient records, inactive the deceased's chart, inactivate patient in the RPMS Patient Registration system or to reactivate retired charts. 7. Knowledge of the Privacy Act of 1974 and the use of confidential information and health records as an integral part of the interviewing office function and the privacy of individuals which must be protected to the fullest extent possible. 8. Knowledge of the Health Insurance Portability and Accountability Act. PERIOD OF PERFORMANCE: The Contractor/Dental Support Assistant will provide service at the White Earth Health Center, Ogema, MN. This position will be used to cover summer vacation and training absences of civil servant employees. Therefore the work schedule will be flexible to accommodate the vacancies as they occur. Duty hours are eight (8) hours per day 8:00 am to 4:30 pm Monday through Friday with a ½ hour lunch break. Contractor shall work up to 5 days a week at 8 hours per day equaling 40 hours per week. Scheduled days will exclude recognized federal holidays; Columbus Day Veterans Day Thanksgiving Day Christmas Day New Years Day M.L. King Day Presidents' Day Memorial Day Independence Day Labor Day This contract will terminate when period of performance expires or if funds availability imposes constraints. REPORTING REQUIREMENTS: Project Officer/Business Office Manager or designated representative shall monitor the Contractor's technical/post progress and promptly inform the Contracting Officer of any problems or potential problems they encounter. The government reserves the right to accept or reject services if the level of work is unsatisfactory. Preference will be given to candidates that have prior experience working in a dental clinic. The "Unit Price" is an all inclusive cost. All "inclusive cost" is defined to include travel, lodging, per diem, fringe benefits, i.e., life insurance, social security, federal, state, and local taxes, plus all other costs pertinent to the performance of this purchase order. "Unit Price" covers hours worked, payment will not be made for holidays or clinic closures. All invoices must have the purchase order number, federal tax ID number, DUNS number, invoice number, invoice date, and period that the invoice covers printed on each invoice for payment to be processed. Failure to comply will stop payment until a corrected invoice is received. Invoice creation is the responsibility of the contractor, not the White Earth Health Center staff. Delays in payment due to invoice errors are the responsibility of the vendor.
