- This is a sources sought notice (Request for Information). The Government intends to award multiple Blanket Purchase Agreements (BPA) for various Surgical Instruments for Ophthalmology (see attached list which includes BUT NOT LIMITED TO items) at the ASFI website: https://acquisition.army.mil. Manufacturer: Alcon Laboratories, 6201 S. Freeway, Ft. Worth, TX, 76134-2001, Phone: 800-862-5266. Calls will be placed against the BPAs as supplies are required. Contractor shall provide all supplies and transportation necessary to Tripler Army Medical Center, Hawaii. Period of performance is 26 February 2011 through 25 February 2016. NAICS is 339112, All Surgical and Medical Instrument Manufacturing. TELEPHONE REQUESTS WILL NOT BE HONORED.
