SOURCES SOUGHT: THIS IS ONLY A SURVEY OF THE MARKET TO DETERMINE IF THERE ARE POTENTIAL CONTRACTORS THAT CAN SATISFY THE FOLLOWING REQUIREMENT: The Department of Veterans Affairs, Bruce W. Carter, VA Medical Center, Miami. FL is seeking vendors interested in competing for an acquisition set aside for Service-Disabled Veteran-Owned Small Business Concerns (SDVOSB). The estimated value of this procurement is between $1,000,000.00 and $2,000,000.00. The applicable North American Industry Class System Code is 339115. The small business size standard is 500 employees. The solicitation in its entirety and any amendments will be available at no charge online at http://www.fbo.gov under the solicitation number VA-248-11-RP-0145. The Contractor shall accept prescriptions authorized by Prosthetics for fabrication of eyeglasses. Prosthetics Program Support Assistants have the authority to approve consults for the procurement of eyeglasses for eligible beneficiaries based on medical justification and prescription. The Contractor shall accept authorization for consultation for eye exams by the Fee Basis Department. The Contractor shall fabricate all eyeglasses in strict conformance to the eyeglass prescription. All items provided by the contractor shall be new. At the present time, this acquisition is expected to be issued as a total SDVOSB set aside. To be considered as a SDVOSB for this acquisition your firm must meet the requirements in accordance with FAR 19.1403. If your firm meets the requirements in accordance with FAR 19.1403 and your firm is interested in submitting an offer on this acquisition, please respond by e-mail (ONLY) to Ogniel.Alvarez@va.gov with "Sources Sought for Solicitation Number VA-248-11-RP-0145" in the subject line. Please reference the above solicitation number in all correspondence pertaining to this project. Your response should include the following: (a) a positive statement of your intent to submit a bid for this solicitation as a prime contractor; (b) a statement identifying your certified SDVOSB designation. Contractors are reminded all work on this contract shall be performed by a Contractor, Licensed in the State of Florida and all work must be in compliance with all applicable industry standards and the Joint Commission.
