- Description
- The Government has a requirement for Mortuary Services for Buckley AFB for handling the remains of authorized personnel. The required services are commercial in nature, therefore, FAR Part 12, Acquisition of Commercial Items shall apply to this acquisition. The US Air Force will award a Mortuary Services Blanket Purchase Agreement (BPA) only to fully qualified professional funeral directors. The contractor shall provide facilities, equipment, licensed personnel, motor vehicles, professional services, supplies, ten death certificates per death, caskets, urns, outer shipping containers, transportation, and perform hygienic practices as specified in this Performance Work Statement. The contractor shall comply with the highest industry standards in performing the following. The Period of Performance is in1 year increments 2. Set-Aside: TBD 3. NAICS Code: 812210 4. All questions or comments must be emailed to SrA Brian Powell, wendy.farnsworth@buckley.af.mil, Contract Specialist, by 01 Oct 2010 at 2:00 pm MST. Telephone and other means of oral communication shall not be permitted. 5. Responses shall include: (1) Company name, address, point of contact, phone/fax/email, and business size and status (e.g., small business, 8(a), veteran owned small business, service-disabled veteran owned small business, HUBZone small business, small disadvantaged business, and women owned small business), DUNS number, and CAGE Code. (2) Proof of experience and capability of meeting requirements in PWS. Capability statements will not be accepted after the due date. No facsimile transmissions will be accepted. CAPABILITY STATEMENTS MUST DEMONSTRATE THE MINIMUM REQUIREMENTS OUTLINED in Statement of Work. 6. The Government will not pay for any material provided in response to this market survey nor return the data provided. 7.
