Attachment 3 - Potential Service Locations in Puerto Rico and Virgin Islands Attachment 2 - Virgin Islands Prevailing Wage Determination Attachment 1 - Puerto Rico Prevailing Wage Determination Solicitation Standard Form 1449 The Social Security Administration (SSA) is seeking quotations for the attached Request for Quotation (RFQ) for the establishment of a Blanket Purchase Agreement (BPA) in order to, on an as needed basis, acquire on-site document shredding services for SSA offices located in Puerto Rico and U.S. Virgin Islands. This notice is a combined synopsis/solicitation for commercial services (as defined by Federal Acquisition Regulation (FAR) 2.101) prepared in accordance with the format in FAR Part 12 and FAR Part 13, as supplemented with additional information included in this notice. This acquisition is being conducted in accordance with FAR Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures. All responsible business sources may submit a quotation, which shall be considered. The solicitation number is SSA-RFQ-R02-11-1004. This is a small business set aside. The North American Industrial Classification Code for Document Shredding Services is 561990, with a small business size standard of $7.0 million. OBJECTIVE: The objective of this solicitation is to obtain firm fixed pricing for a base period plus four one-year ordering periods, for on-site document shredding services, on an as needed basis, for SSA offices located in Puerto Rico and Virgin Islands. Individual call orders will be placed against the BPA. For each call order placed against the BPA by a SSA office, contractor shall provide all labor, personnel, equipment, supplies, secured vehicles, materials, supervision, and other supplies and services necessary to provide on-site secure document shredding service. Contractor is required to recycle all material received from the Government. All contractors must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the contractor is not registered in CCR, it may do so through the CCR website at http://www.ccr.gov. All contractors must undergo a security background screening prior to the performance of any work under the BPA.
