SOURCES SOUGHT. This is a sources sought announcement only! This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of potential qualified sources such as 8(a), HubZone, small disadvantage, service disabled veteran-owned business, and small business relative to NAICS 621512, size standard $13.5M. Responses will be used to determine appropriate acquisition decisions. The Department of Veterans Affairs, Birmingham VA Medical Center (BVAMC), 700 South 19th Street, Birmingham, AL 35233 is seeking sources for a potential contract to provide Radiological Services, and/or Mammography and/or MRI for eligible veterans referred from the Birmingham VA Medical Center (BVAMC) and Community Based Outpatient Clinics (CBOC). For Radiology Services: The contractor�s facility must be within 15 miles of the CBOC VA facility that the contractor proposes to provide radiologic services or 5 miles of the nearest facility if one or fewer facilities are within 15 miles. The addresses are the following: Anniston/Oxford Community Based Outpatient Clinic, 96 Ali Way Creekside South, Oxford, AL 36203; Childersburg Community Based Outpatient Clinic, 151 9th Avenue N. W., Childersburg, AL 35044; Decatur/Madison Community Based Outpatient Clinic, 8075 Madison Blvd., Suite 101 Madison, AL 35758; Gadsden Community Based Outpatient Clinic, 206 Rescia Drive, Gadsden, AL 35906; Huntsville Community Based Outpatient Clinic, 301 Governor's Drive, S.W., Huntsville, AL 35801; Jasper Community Based Outpatient Clinic, 3400 Hwy 78 East, Suite 215, Jasper, AL 35501; Muscle Shoals Community Based Outpatient Clinic, 422 DD Cox Blvd., Sheffield, AL 35660; Guntersville Community Based Outpatient Clinic. 101 Judysmith Drive, Guntersville, AL 35976. For Mammography Services: provided are in the Birmingham area and/or within 15 mile radius of the CBOCs. For MRI Radiological Services : Services provided will be in the Birmingham area and/or within 15 mile radius of the CBOCs and other outlined areas as needed. The intended contract is anticipated as a one base year, with 4 option years. Responses to this sought must include 1. Point of contact. 2. Company Name, 3. Address, 4. Telephone & Fax numbers, 5.
