- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a request for quotation (RFQ) and the solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-48. When submitting offers for this requirement cite the solicitation number W81XWH-11-T-0117. This acquisition is unrestricted and the North American Industry Classification System (NAICS) is 334413. The busiiness size standard is 500 employees The contractor shall provide the following brand name or equal item: CLIN 0001, QTY 1, UNIT OF ISSUE (U/I) EA Part Number 1117782 Chamelon System: Ultra (II) 80 MHz (RoHS) in accordance with the attached minimum essential characteristics; CLIN 0002 QTY 2, U/I EA Equipment Operator Manuals CLIN 0003 QTY 1, U/I EA Service Manual CLIN 0004 QTY 1, U/I EA Installation; and CLIN 0005 QTY 1 U/I EA Training. This acquisition is FOB destination and delivery is required within 30 days after receipt of order to: US Army Institute of Surgical Research, 3400 Rawley E. Chambers Ave., Fort Sam Houston, TX 78234-6315. The following provisions and/or clauses apply to this acquisition.
