- This is a combined synopsis/solicitation for services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is issued as a request for quote (Reference number W881AA-0344-0002). The solicitation document and incorporated provisions and clauses are those in effect the Federal Acquisition Circular 2005-47 and the Defense Federal Acquisition. Regulation Supplement (DFARS) change notice 20101208. It is the contractor's responsibility to become familiar with applicable clauses and provisions. This solicitation is 100% SMALL BUSINESS, NAICS CODE 621210. The date, time and request for quote offers are due by: 1:00 P.M. local time on 21 DEC 2010 to fredesvin.quintana@us.army.mil or lynn.m.williams@us.army.mil This requirement is for the following items: CLIN 0001 - DENTAL TREATMENT SERVICES FOR APPROXIMATLEY 50 SOLDIERS FOR 8 AND 9 JAN 2011 AT BLDG 3106 FT LEWIS, WA. SEE ATTACHED SOW. PLEASE PROVIDE PRICING FOR NUMBER OF TREATMENT TEAMS PER DAY. NUMBER OF TEAMS PER DAY $______________ FOR 2 DAYS $_______________ TOTAL CLIN 0002 CMR CONTRACTOR MANPOWER REPORTING (SEE PARAGRAPH 4.0 OF THE SOW FOR PROCEDURES) $________________ EA VENDOR MUST PROVIDE ADEQUATE PERSONNEL TO SERVE THE NUMBER OF SOLDIERS IN THE TIME SPECIFIED.
