- Contractor shall provide use of Laser Equipment and Certified Technician for the Urology Operating Room at the Department of Veterans Affairs Medical Center (VAMC), Corner of Veterans Way & Lamont Streets, Mountain Home, TN 37684 in accordance with all terms, conditions, provisions, and the schedule of this solicitation. The required equipment shall be as follows: a) Holmium: YAG Laser (22 & 100 watt), b) CO2 Carbon Dioxide Laser, c) Green Light Laser. This procurement is set-aside 100% for small business. NAICS: 621999. Solicitation will be available on/about 27 January 2011. All questions must be in writing and may be emailed to james.bales@va.gov or faxed to 615-225-5432. Email is preferred. No web based offers will be accepted. The following clauses and provisions plus addendum will apply: 552.212-1; 52.212-2; 52.212-3; 52.212-4; and 52.212-5.
