Description
The Department of Veterans Affairs (DVA), VA Caribbean Healthcare System (VACHS), Network Contract Activity,SERVICE AREA OFFICE (SAO) East 8, Tres Rios Building, San Patricio, 27 Ave. J. Gonzlez Giusti Suite 600, Guaynabo, PR 00968 intent to solicit offers and award one firm-fixed price contract on a competitive basis for Hoods Certification applicable North American Industry Classification System (NAICS) code number 541380 and Size Standard is $11.0M. The method of Award will be CASCADE PROCEDURE. Proposal Evaluation will be using a 4-Tier cascading set-aside procedure as follows: First tier: SDVOSB, Second Tier: VOSB, Third Tier: 8(a) and/or HUBZone, Four Tier: All other Small Businesses. Cascading process will be used in conjunction with FAR Part 15, Lowest Price Technically Acceptable (LPTA) source selection process. Award will be made to a responsible offeror on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors; only were adequate/sufficient competition at a given tier is received. The government will rank order the price proposals from lowest to highest at each tier in which adequate competition is received. Award will be made at the highest tier with adequate competition. If the lowest priced offer receives an œAcceptable Technical rating, then the evaluation process concludes and the government will make preparations for contract award. If the lowest priced proposal is not rated œTechnically Acceptable , then the evaluation process continues until a technically acceptable proposal is determined.This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12. The solicitation documents, all incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-27. The DVA intends award a Contract with a base period from the date of award to September 30, 2011, including four (4) one (1) year renewal options period to be exercised at the discretion of the Government. The qualified Contractor shall provide all necessary labor, material and supervision for Hoods Certifications providing inspections, certifications and report of mentioned equipment located at the VA Caribbean Healthcare System, Mayaguez OPC and Ponce OPC. QUESTIONS: shall be submitted in writing via facsimile or e-mail attention to the Contracting Officer no later than 10 days after the solicitation is posted at Fedbizopps (FBO) web site: http://www.fedbizopps.gov/. VERBAL OR TELEPHONE INQUIRIES WON'T BE ACCEPTED. All responses will be answered and incorporated in to a written amendment to the solicitation, for the benefit of all interested offeror's. Solicitation No. VA-248-11-RP-0024 electronic solicitation package will be available to the public as Request for Proposal at FBO web site by or after December 17, 2010. FAXED and E-MAIL PROPOSALS WILL NOT BE ACCEPTED. Offerors shall go to FBO web site to download subject solicitation package, including any related attachments and/or amendments after December 17, 2010. In case the solicitation due date is extended, all interested offeror's shall be responsible to monitor the changes posted at FBO web site, prior the solicitation due date. Each offeror shall acknowledge receipt of each amendment, submitting each amendment attention to the CO at Department of Veterans Affairs, VA Caribbean Healthcare System, NCA Service Area Office East 8, Tres Ros Building - San Patricio, #27 Ave. J. Gonzlez Giusti Suite 600, Guaynabo, PR 00968 by the solicitation due date. The contract resulting from this procurement will be awarded to the responsive and responsible offeror whose offer, conforming to the solicitation requirements will be most advantageous to the Government, in terms of price standpoint, technical factors and past performance. Offer shall includes a fixed firm price for the services shown in the Schedule of Price Section of the solicitation, or offer will be unacceptable to the Government based on the solicitation terms and conditions. There will be no public bid/offer opening meeting on the solicitation due date. The Government reserves the right to award without discussions; therefore, the offeror shall submit the most favorable terms and technical information with their offer. All interested offeror's shall be registered in the Central Contractor Registration (CCR) database before submit their offer. Contractors are responsible for the accuracy of information added to CCR. To be registered, you must go to http://www.ccr.gov/. Current contract information may be re-quested and disclosed under Freedom of Information Act (FOIA), in accordance with 38 Code of Federal Regulations (CFR) 1.553(b). FOIA request shall be addressed attention to Ms. Rosayma Lopez, Privacy Officer, Department of Veterans Affairs, VA Caribbean Healthcare System, 10 Calle Casia, San Juan, PR 00921-3201. All requests shall be in writing by any firm or person, including Requestor's signature, Sender address and print name. Copy of FOIA request may be faxed to the CO for tracking purposes only, but all follow up shall be done with the Privacy Officer. Each request shall reasonably describe the records, so that they may be located with a reasonable amount of effort. Also each FOIA Requestor shall states their willingness to pay applicable fees or provide justification to support a fee waiver. A day time telephone number is required. All envelopes and correspondence must state Freedom of Information Act (FOIA) Request. Discussions are not allowed, all questions shall be send attention to the Contracting Officer for the benefit of all interested parties.
