Description
Solicitation for Lab Pick up and Testing, Base year plus 4 Option years. This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation is available on the FBO website. The Solicitation Number W912PQ-11-T-0002 is being issued as a Request for Quote. The solicitation documents and incorporated provisions are those in effect through Federal Acquisition Circular 05-26. This solicitation is set aside for 100% small business. The cost magnitude is estimated to be $25,000 and $100,000. The NAICS code applicable to this RFP is 621511. The small business size standard is $13,500,000. Under this solicitation, the contractor will provide for pickup of samples between 0900 to 1800 hours, Monday thru Sunday from various sites throughout New York State as designated by NYARNG Medical Command to approximately 2,500 New York Army National Guard Personnel. Once samples are picked up, they will be taken to the Contractor's Laboratory and be processed/ tested then the results will be provided to the NYARNG Medical Command in written format. Contractor must provide written test results to NYARNG MEDICAL COMMAND within 2 business days of receipt of samples through a secure website and by mail/ fax. The place of performance is multiple locations throughout New York State. The period of performance is 1 Jan 2010 through 30 September 2011 for the base year, with the Option years from 1 Oct through 30 Sep for the follosing option years. All technical requirements pertaining to this solicitation are contained in the Statement of Work and the Pricing Sheets for the Lab Tests for the Base year plus 4 Option years. The provision at 52.212-1, Instructions to Offerors - Commercial applies to this solicitation. Award will be made to the lowest offeror, that is technically acceptable. The provision at 52.212-3, Offeror Representation and Certifications applies to this solicitation. Interested parties must complete the Representation and Certifications at the ORCA website. BACKGROUND: Individual medical preparedness is vital for force health protection. Laboratory testing for G6PD deficiency, Lipid Panels and other blood work are required by DoD and NYARNG. 2. OBJECTIVES AND SCOPE: The objective of this Statement of Work is to provide parameters and criteria for the completion (pick up and testing) of Blood/Urine samples as required for Soldiers of the New York Army National Guard (NYARNG).
