- Description
- Q -- Physician Coverage at Santa Fe Indian Hospital including surrounding facilities which include Santa Clara Health Center, Cochiti, Santo Domingo and San Felipe Health Clinics. The Albuquerque Area Indian Health Service has a requirement for coverage of a physician at the Santa Fe Indian Hospital located in Santa Fe, New Mexico and surrounding facilities. Estimated maximum coverage will be under Urgent care shifts, 8:00 pm to 8:00 am, and on as needed basis at the Santa Fe Indian Hospital, 5 shifts per month. The Health Center and Clinics coverage will be on as needed basis, 8 hour shift, 8 am to 4:30 pm, Monday through Friday, up to 5 shifts per month. Plus 8 hour daytime shift 8 am to 8 pm, 12 hour daytime shift 8 am to 8 pm, 12 hour overnight shift 8 pm to 8 am. These shifts will total 40 hours/week of 80 hours/pay period and will include some weekend and federal holiday shifts. Offor must be licensed to practice medicine in any one state of the United States; have graduated from an accredited medical school within the United States. This will be for a multiple contract award with a four one year option renewal. The Government intends to award a IDIQ type contract. A best value concept will be applied for evaluation of offers for this procurement. The best value approach provides the opportunity to select an offeror who is not necessarily the lowest-priced or the highest technically-ranked. The Indian health Service will select the proposal that represents the best value based on the evaluation of technical factors and price. Technical factors consist of past performance and technical capability. Technical factors when combined are approximately equal when compared to cost or price. Therefore, the offeror's proposal should contain the offeror's best terms All interested offerors must have a Dun and Bradstreet Numbering System Number (DUNS), to obtain one call 1-866-705-5711 or by e-mail at www.dnb.com, and be registered in the Central Contractors Registration (CCR) call 1-888-227-2423 or e-mail at www.ccr.gov. Offer must be established providers under an LLC, Inc. or other allied medical organizations. Without these requirements the Government cannot award a contractor to the successful offeror. The Offerors are required to submit, by US Postal Service or hand delivered, hardcopies of their proposals to the following address: Albuquerque Area Indian Health Service, Division of Contract and Grants Management, 5300 Homestead Road NE, Albuquerque, New Mexico 87110. The RFP due date is close of business 4:30 pm MST by January 14, 2011. Late proposals will not be accepted unless they were mail via USPS and post marked prior to the time and date of the due date.
